gnrha在子宫肌瘤剔除术前的临床应用(附20例分析)

Abstract:
目的探讨促性腺激素释放激素激动剂(gnrha)在子宫肌瘤剔除术前的临床应用价值及其对妊娠结局的影响。方法回顾分析我院6年来20例行子宫肌瘤剔除术患者的基本情况,观察术前应用gnrha治疗后子宫及肌瘤的体积变化,随访术后肌瘤复发情况,受孕时间、受孕后有无自然流产、术后分娩时间及在分娩时有无先兆子宫破裂、子宫破裂等情况。结果应用gnrha治疗后临床症状明显好转,子宫及其肌瘤体积明显减少,肌瘤体积缩小近50%,术后有1例复发。妊娠后未发生自然流产,平均孕周(34.0±3.5)周,1例于33周发生先兆子宫破裂,围产儿预后良好,无并发症。结论应用gnrha可纠正贫血,减少肌瘤复发率;能明显缩小肌瘤和子宫体积,使子宫切口小、瘢痕小,减少术时出血,减轻子宫内膜损伤,改善受孕条件,避免大肌瘤子宫全切和孕期子宫破裂的风险;延长了孕周,减少了早产的发生。
