20例原发性脑脓肿立体定向治疗的临床观察

Abstract:

目的探讨立体定向引流技术治疗原发性脑脓肿的临床效果。方法回顾分析20例原发性脑脓肿患者立体定向治疗资料,脓液进行细菌培养病原体分析。结果本组病人无1例死亡,术前颅内压增高者症状很快消失,2例术后发生癫痫。随访1~15年,所有患者均未出现脓肿复发,局灶性神经功能缺损者病例症状完全消失。16例吸取脓液的细菌培养结果为阴性4,例培养阳性患者分别是葡萄球菌、β-溶血性链球菌、肺炎球菌和非溶血性链球菌。结论无菌的脑脓肿即使没有存活的细菌仍然可能表现出自我维持增长的病变趋势。立体定向引流治疗可通过吸取病灶内壁周围的感染刺激因子而达到治疗目的。
