肌电图评价不同手术入路治疗脊柱胸腰段骨折对椎旁肌功能的影响

Abstract:

目的通过肌电图检测观察不同手术入路治疗脊柱胸腰段骨折远期椎旁肌功能的变化及对患者生活质量的影响。方法随访到的胸腰段骨折术后取出内固定12月以上的患者69例,依据取出椎弓根螺钉内固定术的手术方式分组,微创组共37例行椎旁肌间隙入路,传统组共32例行传统后正中入路。随访时两组均进行椎旁肌肌电图检测及SF-36量表评分、Oswestry功能障碍指数评分(ODI评分)。结果两组患者ODI评分微创组明显低于传统组(P<0.05);SF-36评分微创组明显高于传统组(P<0.05);两组患者肌电图检查结果比较,静息状态微创组自发电位的发生率明显低于传统组(P<0.05);轻收缩状态微创组运动单位电位时限增宽和数量减小的发生率明显低于传统组(P<0.05);最大用力状态微创组单混相、单纯相的发生率明显低于传统组(P<0.05)。分别对两组异常和正常电位患者的ODI功能障碍指数评分及SF-36量表评分对比表明,两组中正常电位患者的ODI评分均明显低于异常电位患者(P<0.05),SF-36评分均明显高于异常电位患者(P<0.05)。结论椎旁肌肌电图检测可客观评价椎旁肌功能状态;椎旁肌的功能对患者术后生存质量有一定影响。
