妊娠早期不良暴露与妊娠结局关系的研究

Abstract:

目的分析妊娠早期不良暴露与妊娠结局的关系。方法采取队列研究,对入选对象进行问卷调查并电话追踪随访。记录孕妇年龄、BMI、孕早期TORCH感染史、主被动吸烟史、环境影响因素、心理生理因素及妊娠结局等,对可能影响妊娠结局的相关因素进行单、多因素分析。结果846例研究对象中出现不良妊娠结局99例,不良妊娠结局发生率为11.70%。单因素分析发现,BMI、年龄、流产史、孕早期TORCH感染史、主、被动吸烟史、环境有害因素接触史、情绪异常和不良精神刺激共9个因素间不良妊娠结局的发生率差异均有统计学意义(P<0.05);经多因素非条件Logistic回归分析,产妇年龄(OR=1.778,95%CI=1.077~4.872)、频繁流产史(OR=2.469,95%CI=1.108~7.291)、孕早期TORCH感染史(OR=2.136,95%CI=1.020~4.879)、主动吸烟史(OR=1.421,95%CI=1.007~5.816)和不良精神刺激(OR=1.624,95%CI=1.121~6.019)为孕妇发生不良妊娠结局的主要妊娠早期危险暴露因素。结论妊娠早期不良暴露可增加不良妊娠结局的风险,孕期保健应从孕早期开始。
