创伤后血糖水平对急性伤口感染的影响分析

Abstract:

目的?了解创伤后血糖水平对于急性伤口感染的影响。方法?采用横断面调查研究,通过方便抽样的方法选择2012年7月-10月入住骨科的患者进行调查分析。纳入标准为年龄≥18周岁,住院时间≥3d,入院时已有急性伤口存在或入院后进行手术治疗的患者。共576例患者纳入研究。结果?576例患者中73例发生伤口感染,发生率为12.7%,其中19例患者的伤口感染属于手术切口感染,发生率为3.3%。研究对象受伤后首次测量的血糖值为3.24~22.30mmol/L,中位血糖值5.68mmol/L。伤口发生感染的患者中位血糖值为6.36mmol/L,而伤口未感染者中位血糖值为5.67mmol/L,差异有统计学意义(P<0.001),多因素分析显示血糖水平是伤口感染的独立危险因素(OR=1.592,P<0.001)。结论?创伤后高血糖水平是导致伤口感染的危险因素。
