首诊357例系统性红斑狼疮临床分析

Abstract:

目的?分析357例首次确诊的系统性红斑狼疮(SLE)患者的各种临床表现和实验室检查结果,为SLE的诊断提供参考。方法?收集2008年-2012年期间首次确诊的SLE患者357例,分析患者的临床表现和实验室检查结果对SLE的意义。结果?患者临床表现各异,多系统损害情况明显。所有首次确诊的357例患者中,各脏器受累以关节炎最多,有211例(59.1%);其次是皮疹、肾脏损害。免疫学检查方面,75.9%的患者补体下降。可提取性核抗原谱检查中,抗SSA抗体阳性率最高(51.3%),其次是抗u1RNP抗体阳性(44.0%),随后是抗ds-DNA抗体阳性和抗核小体抗体阳性。首次发病的临床表现最多是皮疹(43.4%),随后为关节炎、下肢水肿;少数以头痛、肢体麻木为首发症状。结论?SLE的临床表现多样,多系统损害明显,异质性表现突出。
