益生菌预防老年抗生素相关性腹泻的Meta分析

Abstract:

目的?评价益生菌预防老年人抗生素相关性腹泻的疗效。方法?计算机检索PubMed、EMbase、WebofScience、Cochrane系统评价数据库、Cochrane临床对照试验注册库、中国知网、维普、万方,并手工检索相关参考文献等,检索时间跨度为1966年1月-2014年4月。所纳入研究均为随机对照试验,研究对象≥65岁,研究内容为抗生素使用时或使用前预防性使用益生菌,文献Jadad评分须>3分。结果?共纳入8篇文献,包含3680例研究对象(益生菌组1843例,对照组1837例)。Meta分析结果显示益生菌组与对照组间发生抗生素相关性腹泻(AAD)的风险差异无统计学意义RR=0.76,95%CI(0.51,1.13),P=0.17。亚组分析发现乳杆菌属RR=0.59,95%CI(0.31,1.13),P=0.11、酵母菌属RR=1.24,95%CI(0.70,2.19),P=0.46、多种益生菌联合RR=0.58,95%CI(0.24,1.41),P=0.23与对照组相比,差异均无统计学意义。结论?尚无证据支持益生菌能够降低老年人服用抗生素后发生AAD的风险。
