髓核突出导致大鼠痛觉改变及背根神经节交感神经芽生

Abstract:

目的观察单纯自体髓核突出情况下是否可以造成背根神经节(dorsalrootganglion,DRG)内交感芽生现象的发生,并引起大鼠痛阈变化。方法雄性SD大鼠110只,按随机数字表法分为正常组、手术组和假手术组,正常组10只,手术组、假手术组术后2、5、7、14、20d5个时间点,各时间点10只。手术组将鼠尾近端椎间髓核取出后移植于大鼠L5DRG表面,制成无机械性压迫的单纯髓核突出模型。假手术组仅通过手术暴露L5DRG,并不进行髓核移植。不同时间点监测大鼠的机械性痛阈与热痛阈,并处死、灌注固定。将固定后的术侧DRG取出、切片,用酪氨酸羟化酶(TH)抗体进行交感神经免疫组化染色,光学显微镜拍照后通过图像处理软件检测DRG内交感神经的数量、密度。结果手术组大鼠自体髓核移植术后,有明显行为学改变,大鼠术侧后足机械性痛阈和热痛阈明显降低,术后7、14、20d痛阈水平均明显低于假手术组和正常组(P<0.05)。同时,手术组术侧DRG内交感神经数量在术后5、7、14、20d均显著高于假手术组和正常组(P<0.01)。术后20d,手术组DRG内交感神经数量[(19.70±2.67)mm/mm2]明显高于假手术组[(2.75±2.20)mm/mm2]和正常组[(1.11±1.07)mm/mm2,P<0.01]。结论大鼠通过单纯自体髓核刺激DRG即可引起DRG内交感神经芽生,并可以引起同侧肢体痛阈降低。
