Carcinoma del tiroides: Resultado del tratamiento quirúrgico

OBJETIVOS: Analizar la experiencia en el tratamiento quirúrgico del carcinoma tiroideo. MéTODOS: Se evaluaron 85 pacientes con carcinoma de tiroides tratados en el Servicio de Cirugía General No 1, del Hospital Universitario Miguel Pérez Carre o, entre octubre de 1970 y 2003. Se analizaron: la clínica, diagnóstico, tratamiento, complicaciones y seguimiento. La información se procesó con el paquete estadístico BMDP. Los resultados se expresan en: distribución de frecuencia, estadísticas descriptivas. Se utilizó el Chi-cuadrado; la prueba "t" para valorar diferencias entre promedios. La sobrevida con la prueba de Mantel-Cox, valor de P= 0,05. RESULTADOS: El diagnóstico histológico fue papilar en el 64,70 %, folicular en 17,64 %, medular en el 10,58 %, indiferenciado en el 7,05 %. En 76 pacientes, un nódulo palpable fue el único signo de enfermedad. Edad promedio fue 48 a os. La variedad papilar se presentó en pacientes más jóvenes que el folicular (P= 0,03). Prevaleció en el sexo femenino en las menores de 49 a os (3,6:1) comparado con (2,3:1) en las pacientes > de 50 a os. La gammagrafía reveló un 74 % de tumores no captantes. La ultrasonografía demostró un nódulo solitario en el 96 % de los casos. La tiroidectomía total fue realizada en el 82 %, hemitiroidectomía o tiroidectomía subtotal en el 12 %, los restantes recibieron tratamiento paliativo. La recurrencia alcanzó un 6 %. CONCLUSIONES: Es necesario realizar estudios prospectivos aleatorizados para mejorar los conocimientos sobre la evolución y tratamiento del carcinoma del tiroides. SUMMARY OBJECTIVES: To analyze the experience in the surgical treatment of the thyroid carcinoma. METHODS: We evaluated 85 patients with carcinoma of thyroids treated in the Service of General Surgery No 1 of the University Hospital Miguel Perez Carre o, between 1970 and 2003. The results are expressed in: frequency allocation, descriptive statistics. Comparisons of groups were by the Chi-square test, and Student t- test was used to value differences between averages. The survival curves were compared by the Mantel-Cox test. RESULTS: The histological diagnoses were papillary 64.70 %, follicular 17.64 %, medullar 10.58 %, undifferentiated 7.05 %.
