Comparison of the clinical outcomes of Karydakis ap versus primary closure procedures in the surgical treatment of sacrococcygeal pilonidal sinus disease

Abstract:

Objective: The aim of this study is to compare the clinical outcomes of Karydakis ap versus primary closure (PC) which are two often applied surgical procedures in the treatment of sacrococcygeal pilonidal sinus disease. Material and Methods: The patients who underwent Karydakis ap and PK procedures with the diagnosis of sacrococcygeal pilonidal sinus disease between January 2007 and December 2010 were retrospectively analyzed in a prospectively collected database. Chi-square and t test was used for statistical studies. Results: 130 patients were operated during the study period. PC was the procedure of the choice in 62 (47.7%) and Karydakis ap was performed in 68 (52.3%) patients. Following PC, 4 (6.5%) patients had infections, 14 (22.6%) patients had wound disruption. These complications were seen in 3% and 0, in Karydakis group, respectively (p<0.05). Hematoma was detected in 12 (9.2) patients and distribution between the groups was similar (p>0.05). At the end of a year, recurrence rates were 16.1% and 2.9% for PC and Karydakis, respectively (p<0.05). Conclusion: In the surgical treatment of pilonidal sinus disease, Karydakis ap procedure is a more effective method than PC with lower complication and recurrence rates.
