The ultrasonic manifestation and misdiagnosis analysis of ovarian endometrial cysts

Abstract:
Objective To investigate sonographic features of ovarian endometrial cysts (OEC) and diagnosis value of two-dimensional and color Doppler flow imaging and analyse the reasons of misdiagnosis. Methods A total of ninety-three (107)cases patients with pathologically proved OEC were retrospectively evaluated by the sonographic images and pre-operation diagnosis. Results The 107 OEC sonographic repre- sents were classified as six types: type 1 : 3 normal ovary types were shown as small anechoic zones within a littler big ovary; type 2:11 simple cyst types were shown as well-defined anechoic zones; type 3:28 homogeneous dot hyperechoic inside cyst types were shown as anechoic zone with homogeneous dot hyperecho; type 4:29 multi-bursa types were shown as anechoic zones with zonal interval or dot hyperecho; type 5:33 mixed types were shown as hyperechoic anechoic zones; type 6:3 solid types were shown as anterior low echo zone with hyperecho. The type 3 with relative typical features had higher diagnostic accurgcy ( 92.9%, 26/28 ). The other types were difficult to be diagnosed. Forty-two cases in 107 OEC were misdiagnosed as 15 ovary cysts, 7 cystadenomas, 2 teratoma, 6 ovarian tumors, 2 subserous myomas, 2 inflammatory masses, 5 nonconfirmed masses, 3 little big ovary or liquidtic lesions before operation, respectively. Conclusions The relative typical features of type 3 was shown as anechoic zone with homogeneous dot hyperecho and well-defined and had higher diagnostic accuracy. The other types were difficult to be diagnosed and should be differ entiated from other ovary lesions, such as ovarian simple cyst, teratoma, cystadenoma, subserous myoma and inflammatory mass in pelvic cavity.
