Abstract: hirschsprung′s disease or primary megacolon is an uncommon condition, is more frequent in male than female. a case of a 21 years-old patient is reported, who consulted at 17 weeks of pregnancy, presenting general discomfort and abdominal pain, who at the moment of admittance has no symptoms of peritonitis, just slight pain on lower abdomen. abdomen radiography showed ditalation of tansverse and descendent colon, with free subdiaphragmatic air, which indicated perforation. exploration during surgery confirmed colonic dilatation of approximately 20 cm of transverse and descendent colon with two perforations in this segment. a sub-total resection of transverse and descendent colon was done. pathology report established absence of nerve cells in auerbach and meissner plexus in all wall thickness of extirpated colonic portion.
