Ruptura esplénica espontánea postrasplante y angiomatosis difusa del bazo

Abstract:

diffuse angiomatosis of the spleen is a rare vascular proliferation. spontaneous rupture is the main potentially lethal complication observed in 25 % of cases. a 48 years-old male black patient with history of malignant blood hypertension, 2 years of hemodialysis treatment and kidney transplantation from a dead donor with compatibility of hla groups with the referred patient was the case presented here. a triple immunosupressive therapy based on imuran, cyclosporine a and prednisone was applied. he presented with late recovery syndrome of the graft function secondary to acute tubular necrosis and/or acute rejection; therefore, he required hemodialysis treatment. after 7 days, he began feeling abdominal pain in epigastrium and left hypochondrium without peritoneal reaction. some time afterwards, the pain was deeper and accompanied by hypotension, weakness in lower limbs, dizziness and peritoneal reaction. further emergency tests showed progressive reduction of hemoglobin figures and ultrasound revealed free-flowing fluid in the abdominal cavity. the abdominal puncture extracted blood, splenic rupture was diagnosed as a cause of bleeding, and emergency splenectomy was performed. the biopsy diagnosed diffuse splenic angiomatosis, probably due to some malformation.
