Tratamiento insulínico intensivo versus convencional de la hiperglucemia en el paciente grave

Abstract:

background: hyperglycemia in patients on critical condition is considered a sign of poor prognosis so it is necessary a strict glycaemic control, where the intensive insulin therapy is most commonly used. objective: to propose a scheme of intensive insulin treatment for glycaemic control in seriously ill patients. method: an observational prospective study was conducted in 240 patients admitted in the critically ill care service at the university hospital manuel ascunce domenech from june 2008 to september 2008, which represented the 22,3 % of the total of patients admitted in these units, 125 women and 115 men, the age group of 66 years and over prevailed. all patients presented two glycemia over 7, 8 mmol/l, in the first 12h of admission were included and according to the primary physician criterion, conventional or intensive hypoglycemic treatment was applied. results: the intensive hypoglycemic treatment was used in 20 % of cases, reporting in a shortest time to achieve glycemic control. the hospital stay in this group was lower than 25 % in comparison with the group that received conventional treatment. conclusions: intensive hypoglycemic treatment proved to be effective in seriously ill patients with hyperglycemia.
