Tooth loss has a number of consequences, including bone resorption that compromises prosthetic retention, as well as psychological impacts. That explains the state of depression and poor self-image in the totally edentulous patient. To overcome these problems, it is always preferable to delay extraction of the last teeth, and exploit them when the indication is well established to create a tooth-supported overdenture. This approach offers psychological and biological benefits such as the maintenance of proprioception, preservation of alveolar bone, as well as improving retention and stability of mandibular complete dentures, thus improving comfort during mastication. This article uses a clinical case to highlight the steps involved in the fabrication of a tooth-supported overdenture with ball attachments at 33 and 43.
