合并脑卒中病史的社区获得性肺炎初始治疗失败的研究

Abstract:
社区获得性肺炎(CAP)是一种有着高发病率及高死亡率的疾病。同样脑卒中的发病率亦在逐年增加,有很多患者均遗留不同程度的后遗症,其并发的肺部感染可能导致卒中后临床结果恶化和死亡率增加。而CAP初始治疗成功与否对患者的预后有着很大的影响。尽管国内外医疗水平在快速发展,但仍有众多研究表明CAP的初始治疗失败率偏高。对于脑卒中患者来说,影响其初始治疗成败的因素有很多,如患者所感染的病原体、初始抗菌药物的选择、合并其他基础疾病及并发症、脑卒中所致后遗症及其他辅助治疗等。需综合分析患者疾病情况,给予合理的初始治疗。本文对合并脑卒中病史的CAP患者初始治疗失败的相关因素进行综述,旨在为合并脑卒中病史的CAP的临床诊治提供参考。
Community-acquired pneumonia (CAP) is a disease with high morbidity and mortality. The incidence of stroke is also increasing year by year, and many patients are left with varying degrees of sequelae, and their concomitant pulmonary infections may lead to worsening clinical outcomes and increased mortality after stroke. The success of the initial treatment of CAP has a great impact on the prognosis of patients. Despite the rapid development of medical care at home and abroad, numerous studies have shown that the initial treatment failure rate of CAP is high. For stroke patients, there are many factors affecting the success or failure of the initial treatment, such as the pathogens infected by the patient, the choice of initial antimicrobial drugs, the combination of other underlying diseases and complications, the sequelae caused by stroke, and other adjuvant treatments, etc. The patient’s disease situation needs to be analyzed comprehensively, and the treatment should be given to the patient. It is necessary to comprehensively analyze the patient’s disease situation and give reasonable initial treatment. This article reviews the factors associated with initial treatment failure in CAP patients with a history of combined stroke and aims to provide a reference for the clinical management of CAP with a history of combined stroke.
