游离股前外侧皮瓣在断肢创面覆盖的应用
Application of Free Anterolateral Thigh Flap to Cover Severed Limb Wound

目的：探讨游离股前外侧皮瓣移植修复断肢皮肤缺损的应用。方法：采用游离股前外侧皮瓣移植修复10例断肢创面修复，皮肤软组织缺损面积为8*5~16*15 cm。结果：10例皮瓣均成活，供区直接缝合或者带蒂皮瓣转移缝合，1例出现静脉危象，急诊行手术探查，可见静脉血栓形成，取出血栓后，皮瓣紫斑逐渐消失，后期成活；9例获得随访，随访时间为6个月~24月，患肢功能满意，部分皮瓣后期行整形获得良好的外观。结论：对于断肢的皮肤缺损，局部转移皮瓣无法覆盖创面，或者瘢痕增生明显，影响外观，游离股前外侧皮瓣是一种理想的治疗方案，也可以取得一个良好的理想结果。
Objective: To investigate the application of free anterolateral thigh flap transplantation in repairing skin defects of severed limbs. Methods: The free anterolateral thigh flap was used to repair the wounds of 10 severed limbs. The skin and soft tissue defect area was 8*5~16*15 cm. Results: All the 10 cases survived, the donor area was sutured directly or the pedicled flap was transferred and sutured. Venous crisis occurred in 1 case, and venous thrombosis was observed by emergency surgical exploration. After the thrombus was removed, the purple spot of the flap gradually disappeared and survived in later stage. 9 cases were followed up from 6 months to 24 months, the function of the affected limb was satisfactory, and some flaps were reconstructed later to obtain a good appearance. Conclusion: For the skin defect of severed limb, the local transfer flap can not cover the wound, or the scar hyperplasia is obvious, affecting the appearance, the free anterolateral thigh flap is an ideal treatment plan, and can also obtain a good ideal result.
