冠状动脉冻结技术在改善较高心率患者冠状动脉图像质量中的应用价值

Abstract:

目的?探讨冠状动脉冻结技术(SSF)在改善较高心率患者64层螺旋CT冠状动脉图像质量中的应用价值。方法?回顾性分析2013年8月-2014年2月行CT成像检查过程中平均心率≥70次/min的患者98例,排除其中有呼吸运动伪影者38例,最后纳入60例患者的冠状动脉图像进入分析研究,分析其标准重建图像及基于SSF平台的运动校正后图像。将图像质量分为4级,按美国心脏协会冠状动脉分段标准评价冠状动脉各个节段以及各冠状动脉分支图像质量,比较运动校正前后冠状动脉各分支及各节段出现伪影数及图像质量评分的差异。结果?共评价180支血管,795个冠状动脉节段。运动校正前共有133支(73.89%)血管,287个(36.10%)节段出现运动伪影,运动校正后共有89支(49.44%)血管,150个(18.87%)节段出现运动伪影(P<0.05);右冠状动脉出现运动伪影的比率高于左前降支及左旋支,分别是91.67%(55/60)、60.00%(36/60)、70.00%(42/60),运动校正前评分也低于左前降支及左旋支,分别是(3.32±0.40)、(3.69±0.42)、(3.44±0.55)分,特别是右冠状动脉的S2段,校正前出现伪影的比率是90.00%(54/60),图像质量评分为(2.45±0.85)分。经运动校正后冠状动脉各节段及各分支出现伪影数均较前减少,图像质量评分也高于运动校正前,特别是右冠状动脉及其各节段图像质量改善明显(P<0.05)。结论?SSF在改善较高心率下冠状动脉图像质量中有价值,特别是对右冠状动脉,图像质量有明显提高。
