264例肺腺癌患者EGFR基因突变及TKI类药物临床疗效分析

Abstract:

目的分析肺腺癌患者EGFR19、21基因突变的发生率,以及野生型和突变型患者对EGFT-TKI治疗的有效率。方法收集2011-2013年在我科诊治的肺腺癌患者病理组织或体液标本264例,采用DNA直接测序法检测EGFR19、21外显子突变情况。回顾性分析进行EGFR-TKI分子靶向药物治疗的患者临床疗效。结果264例标本中有72例(27.3%)被检测出具有EGFR19缺失突变或EGFR21L858R外显子点突变,其中EGFR19缺失突变率和EGFR21L858R点突变率分别是15.1%和12.1%。另发现EGFR19P753L、EGFR21L861Q和EGFR21L861P等少见突变。在接受EGFR-TKI类药物治疗的患者中,突变阳性患者的疾病控制率明显优于突变阴性患者(87.0%vs55.9%,P<0.01),EGFR19缺失突变患者的疾病控制率也明显优于EGFR21L858R点突变患者(94.4%vs60.0%,P<0.01),且TKI治疗毒性小,耐受性良好。结论EGFR基因突变在肺腺癌患者中较为常见,其中以EGFR19外显子缺失和EGFR21外显子L858R点突变最为常见,这类患者对TKI类药物具有更好的临床疗效,特别是EGFR19缺失突变。
