种植支持全牙弓和分段式受力的对比分析

Abstract:

目的建立下颌骨三维有限元模型,探讨无牙颌患者种植支持式固定义齿修复的类型,了解全牙弓和分段式修复体对下颌骨及种植体的受力异同。方法临床采集患者下颌骨锥形束CT(conebeamcomputerizedtomography,CBCT)数据及临床制作标准牙冠数据的扫描文件,使用逆向工程软件建立下颌骨种植支持式全牙弓和分段式三维有限元模型,通过有限元分析软件ANSYS分析正常咬合力作用下下颌骨、种植体以及修复体应力的大小。结果在正常咬合力的作用下,全牙弓式骨皮质和骨松质von-Mises应力均小于分段式;全牙弓式各个种植体von-Mises应力均小于分段式,分段式各个种植体的von-Mises应力比全牙弓式更集中;全牙弓式修复体von-Mises应力大于分段式。结论在生物力学方面,种植支持式全牙弓修复方式比分段式更有利于下颌骨和种植体的保护作用,但是不利于修复体的保护。
