Atención preoperatoria del anciano hipertenso

Se evaluó un programa de atención al anciano hipertenso candidato a cirugía mayor electiva, incluyó valoración clínica, investigaciones, tratamiento y seguimiento hasta el posoperatorio inmediato. Una vez descartada la existencia de disautonomía simpática cardiovascular, se utilizó la prueba de estimulación por frío como prueba física de provocación de hipertensión arterial, método auxiliar en el diagnóstico de control tensional estable y guía en la atención terapéutica. Ninguno de los 1 010 pacientes estudiados fue suspendido. Se concluyó que la aplicación del programa permite una reducción considerable de las suspensiones operatorias por hipertensión arterial en los ancianos. A program for the care of the hypertensive elderly candidate to elective major surgery was evaluated. Clinical assessment, research and follow-up to the immediate postoperative were included in the evaluation. Once the existance of cardiovascular sympathetic disautonomy was discarded, it was used the cold pressor test as a physical test to produce arterial hypertension. This is an auxiliary method in the diagnosis of steady blood pressure control and a guide in therapeutical care. None of the 1 010 patients under study was suspended. It was concluded that the implementation of the program allows to attain a considerable reduction of the surgical suspensions due to arterial hypertension in the elderly.
