Tratamiento quirúrgico de las lesiones de la columna vertebral en pacientes con mieloma múltiple

220 patients with multiple myeloma diagnosis, 8 males and 12 females, with an average age of 54 years old, that were operated on due to vertebral column injuries were studied in order to evaluate the results of the surgical treatment. it was proved that myeloma g and that of bence jones were the most common (9 and 6 patients), and that most of them were in stage iii or ii of durie-salmon (11 and 8, respectively), the dorsal localization was the most frequent (14 cases). it was osteolytic type in 12, whereas 7 had extense destruction and fractures. intralesion recanalization with internal stabilization was performed in 17, stabilization without recanalization in other 2, and laminectom,y in 1. there were complications in 13 cases and of the 18 reported, those with local sepsis, postoperative neuritis and granulomas of the wound were the most frequent. at the time of surgery, acute or disasbling pain was reported in 13, and manifestations of neurological damage were observed in 18. the evaluation carried out a year later showed that 12 (60 %) had no pain, or it was mild, and that 145 (70 %) had no neurological manifestations. it was found that 5 patients died during the first year of the operation due to complications of the myelomatoid disease non related to surgery. one of the dead presented relapse of the myelomatoid injury of the vertebral column. it was concluded that the surgical treatment of the vertebral column injuries in patients with multiple myeloma and painful and/or neurological manifestations may be a beneficial therapeutic possibility.
