Research Progress of 3D Visualization in Hilar Cholangiocarcinoma %A 严积灿 %A 于文昊 %A 任利 %A 卢倩 %J Advances in Clinical Medicine %P 1447-1451 %@ 2161-8720 %D 2023 %I Hans Publishing %R 10.12677/ACM.2023.132200 %X 肝门部胆管癌早期诊断困难,仅有约20%~30%的患者能行R0根治性切除术,预后极差。及时有效的解除胆道梗阻,缓解黄疸是其治疗的主要方式及难点。经内镜逆行胰胆管造影术(endoscopic retrograde cholangiopancreatography, ERCP)通过植入胆道支架,可有效缓解黄疸症状,但术后胆管炎发生率较高,且引流肝脏体积不精确。三维可视化技术可于术前规划预引流胆道走形并测算拟引流肝脏体积,提高内镜胆道引流术的精准度。本文就三维可视化技术在肝门部胆管癌中的相关治疗进展作一综述,以为临床提供实践依据。
Early diagnosis of Hilar cholangiocarcinama is difficult, only about 20%~30% of patients can receive radical R0 resection, and the prognosis is very poor. Timely and effective relief of biliary obstruction, relieving jaundice is the main treatment methods and difficulties. Meanwhile, through the implan-tation of biliary stent, endoscopic retrograde cholangiopancreatography (ERCP) can effectively re-lieve the symptoms of jaundice, but the incidence of postoperative cholangitis is relatively high, and the volume of the liver in drainage is not accurate. Three-dimensional visualization technology can plan the shape of the pre-drained biliary tract and measure the volume of the liver to be drained before surgery, thus improving the accuracy of endoscopic biliary drainage.
