%X 摘要 目的 探讨影响结直肠癌同时性腹膜转移的临床病理的相关因素。 方法 收集1994—2018年我院3 826例结直肠癌患者的临床资料,对其行非条件Logistic回归分析,探讨影响结直肠癌腹膜同时性转移的临床病理相关因素。 结果 3 826例结直肠癌患者中,腹膜转移64例(1.62%)。对其行单因素分析显示,患者性别、肿瘤位置、分化程度、病理学类型及癌胚抗原(CEA)、糖类抗原125(CA125)和CA19-9水平与结直肠癌腹膜转移有关(χ2=18.29~80.48,P<0.05)。多因素Logistic回归分析显示,肿瘤位置、肿瘤分化程度及CEA、CA125、CA19-9水平与结直肠癌同时性腹膜转移有关(P<0.05)。 结论 右半结肠癌、肿瘤分期(晚)、低分化、黏液腺癌或印戒细胞癌及CEA、CA19-9、CA125水平增高均是结直肠癌同时性腹膜转移的危险因素。
Abstract:Objective To investigate the related clinical and pathological factors affecting the synchronous peritoneal metastasis from colorectal cancer. Methods The clinical data were collected from 3 826 patients with colorectal cancer who were hospitalized in our hospital from 1994 to 2018; the data were analyzed using unconditional Logistic regression to investigate the related clinical and pathological factors affecting the synchronous peritoneal metastasis from colorectal cancer. Results Of the 3 826 patients with colorectal cancer, 64 (1.62%) had peritoneal metastases. Univariate analyses showed that patient’s gender, tumor staging, location and pathological type of tumor, degree of tumor differentiation, and levels of carcinoembryonic antigen (CEA), carbohydrate antigen 125 (CA125), and CA19-9 were related to peritoneal metastasis from colorectal cancer (χ2=18.29-80.48,P<0.05). A multivariate Logistic analysis showed that tumor location, degree of tumor differentiation, and levels of CEA, CA125, and CA19-9 were related to synchronous peritoneal metastasis from colorectal cancer (P<0.05). Conclusion Right colon cancer, advanced tumor staging, poor differentiation, mucinous adenocarcinoma, or signetring cell carcinoma, and elevated levels of CEA, CA19-9, and CA125 are risk factors for synchronous peritoneal metastasis from colorectal cancer
