%X We present the case of a 52-year-old man with human immunodeficiency virus (HIV) encephalitis resulting from cerebrospinal fluid (CSF) viral escape, to illustrate therapeutic challenges in patients with emergent CSF genotypic HIV drug resistance. This case report highlights the usefulness of CSF HIV-resistance testing to guide antiretroviral therapy and treatment optimizing decisions
