%X 摘要：目的 探讨NF-E1b和CD276在散发性结直肠癌(sporadic colorectal carcinoma, SCC)中的表达及对其预后的影响。方法 收集焦作市人民医院2009年1月至2012年1月病理科术后石蜡标本136例，采用免疫组织化学法检测NF-E1b、CD276在标本中的表达情况，同时对患者进行随访。分析NF-E1b、CD276的表达情况与SCC临床病理特征及预后之间的关系。结果 NF-E1b、CD276在SCC中表达阳性率分别为66.18%(90/136)和54.41%(74/136)。NF-E1b、CD276的表达情况与患者临床病理特征有关(P<0.05)，NF-E1b阳性表达更多见于分期晚、分化低的患者，更易发生淋巴结转移；CD276阳性表达更多见于分期晚、分化低、肿瘤较小的患者，更易发生淋巴结转移。NF-E1b阳性表达患者5年生存率为21.11%(19/90)，低于阴性表达患者的63.04%(29/46)，差异有统计学意义(P<0.05)；CD276阳性表达患者5年生存率为33.78%(25/74)，低于阴性表达患者的61.29%(38/62)，差异具有统计学意义(P<0.05)。NF-E1b的表达与CD276的表达之间呈正相关关系(P<0.05)。Cox多因素分析显示，NF-E1b、CD276是影响SCC患者术后生存时间的独立因素。结论 NF-E1b、CD276与SCC的临床病理特征有关；NF-E1b、CD276阳性表达患者预后更差；NF-E1b、CD276具有明显的相关性。NF-E1b、CD276可为SCC患者的诊疗及预后预测提供参考依据。<br>ABSTRACT: Objective To explore the expressions of NF-E1b and CD276 in patients with sporadic colorectal carcinoma (SCC) and their effect on prognosis. Methods We collected 136 cases of SCC paraffin specimens in our hospital from January 2009 to January 2012. Immunohistochemistry was used to detect the expressions of NF-E1b and CD276 in SCC. We analyzed the relationship of NF-E1b and CD276 expressions with their clinicopathological characteristics and prognosis. Results The positive rate of NF-E1b and CD276 in SCC was 66.18% (90/136) and 54.41% (74/136), respectively. The expressions of NF-E1b and CD276 were correlated with clinicopathological characteristics (P<0.05). The positive expression of NF-E1b was more likely to be found in patients with late stage and poorly differentiated SCC, and was more prone to have lymph node metastasis. The positive expression of CD276 was found more in patients with late stage and poorly differentiated SCC as well as smaller tumor, and was more prone to have lymph node metastasis. The 5-year survival rate of patients with positive expression of NF-E1b was 21.11% (19/90), which was significantly lower than that of patients with negative expression of NF-E1b (63.04%, 29/46)(P<0.05). The 5-year survival rate of patients with positive expression of CD276 was 33.78% (25/74), which was significantly lower than that of patients with negative expression of CD276 (61.29%, 38/62) (P<0.05). There was a positive correlation between the expressions of NF-E1b and CD276 (P<0.05). COX multivariate analysis showed that NF-E1b and CD276 were independent factors affecting the survival time of patients with SCC. Conclusion NF-E1b and CD276 were associated with clinicopathological characteristics of SCC. Patients with positive expressions of NF-E1b and CD276 have poorer prognosis.
