%X 摘要:目的 观察羟考酮联合右美托咪定在预防瑞芬太尼麻醉后痛觉过敏中的效果。方法 选择60例全麻下行妇科择期开腹手术的患者,随机分为2组,A组:麻醉开始前10min泵注右美托咪定0.5μg/kg,后改为0.4μg/(kg?h)至手术结束停药,手术结束前10min静注羟考酮0.8mg/kg;B组:同样方法泵注等量生理盐水,手术结束前10min静注羟考酮1.5mg/kg;记录所有患者的手术时间、苏醒时间。于拔管后即刻、术后30、60、90min对患者进行Ramsay镇静程度评分、BCS舒适度评分及VAS视觉模拟评分法评估患者疼痛程度,记录术后不良反应及追加镇痛药品情况。结果 A组患者拔管即刻和拔管30min Ramsay镇静评分明显低于B组(P<0.05),2组患者术后VAS评分无统计学差异(P>0.05),A组患者术后BCS舒适度评分较B组高(P<0.05),A组患者术后头晕发生率低于B组,且追加药品人次较B组少(P<0.05)。结论 羟考酮复合右美托咪定预防术后痛觉过敏效果良好,患者舒适度较高。
ABSTRACT: Objective To evaluate the effect of oxycodone combined with dexmedetomidine on postoperative hyperalgesia induced by remifentanil in patients. Methods Sixty patients undergoing elective gynecological laparotomy were evenly divided into two groups using a random number table: dexmedetomidine+oxycodone group (Group A) and oxycodone group (Group B). In Group A, dexmedetomidine of 0.5μg/kg was infused at 10min before induction of anesthesia, followed by continuous infusion at 0.4μg/(kg?h) until the end of operation; oxycodone of 0.8mg/kg was infused at 10min before the end of operation. In Group B, the equal volume of normal saline was given instead; oxycodone 1.5mg/kg was infused at 10min before the end of operation. Operation time and awakening time were recorded. Ramsay score, VAS score, and BCS score were recorded immediately and at 30, 60, 90min after extubation; oxycodone consumption was recorded. Results BCS score was higher in Group A than in Group B (P<0.05). There was no significant difference in VAS score between the two groups (P>0.05). Ramsay score was lower in Group A than in Group B immediately and 30min after extubation (P<0.05). Compared with Group B, the incidence of dizziness and oxycodone consumption were significantly decreased in Group A (P<0.05). Conclusion Oxycodone combined with dexmedetomidine can reduce postoperative hyperalgesia induced by remifentanil much better than oxycodone alone in patients
