%X This article is about transitions in life, more in particular the transitions in health and care that are related to these changes. We will focus on the awareness and the engagement that Meleis distinguished in the patient experience of transition. And how nurses can take clinical leadership to advance patient awareness and engagement and in this way promote self-management. We will focus on the question what the contribution of nurse practitioners may be in coordinating health professionals and promoting collaboration between stakeholders and between the health and the social domain. This is transitioning care.
