%0 Journal Article %T 利用Myrian系统对胰腺周围主要血管进行3D重建<br>3D reconstruction of major blood vessels around the pancreas with Myrian system %A 王 %A 亮 %A 钟 %A 武 %A 陈睦虎 %J 西安交通大学学报(医学版) %D 2017 %R 10.7652/jdyxb201704024 %X 摘要:目的 探讨利用Myrian系统构建胰腺周围主要血管的3D结构的方法及其在临床应用中的意义。方法 通过GE64层CT采集60例胰腺部位扫描数据,利用Myrian软件系统对胰腺周围动脉血管群、静脉血管群的分布情况进行3D重建,并测量血管长度、夹角、汇入情况等数据。结果 腹腔的主要动脉,如腹主动脉、脾动脉、肝总动脉,以及腹腔的部分静脉,如门静脉、脾静脉、肠系膜上静脉的重建率可达100%;门静脉干长度平均(46.37±11.48)mm,其中大于45mm的优势长度占45.54%;胰后干长度为(34.88±9.13)mm,其中大于35mm的优势长度占57.21%。门静脉与脾静脉的夹角为锐角的有17例(占28.33%),为直角的有11例(占18.33%),为钝角的有32例(占53.33%)。肠系膜下静脉的汇入方式为脾静脉的有19例(占31.67%),为肠系膜上静脉的有34例(占56.67%),为脾静脉和肠系膜上静脉交汇点的有7例(占11.67%)。结论 Myrian系统可用于对胰腺周围血管的3D结构重建,而且重建率较高,可用于指导临床手术实施的可行性分析,减少手术过程中因不清楚血管分布情况而导致的出血风险。<br>ABSTRACT: Objective To explore the method of 3D reconstruction of major blood vessels around the pancreas with Myrian system and its clinical significance. Methods By making use of the data from 64-slice CT of GE, the main arteries, portal vein, spleen vein, superior mesenteric vein and inferior mesenteric vein of 60 pancreases were 3D reconstructed with Myrian system. Vessel length and some other data were also recorded. Results The main arteries and veins were reconstructed. The length of portal vein was (46.37±11.48)mm. The length of post-pancreas trunk was (34.88±9.13)mm. The angle between portal vein and spleen vein was acute angle in 17 cases (28.33%), right angle in 11 cases (18.33%) and obtuse angle in 32 cases (53.33%). The inferior mesenteric vein fed into spleen vein in 19 cases (31.67%), superior mesenteric vein in 34 cases (56.67%) and conjunction point between spleen vein and superior mesenteric vein in 7 cases (11.67%). Conclusion Myrian system can be used to reconstruct the 3D structure of blood vessels around the pancreas with high reconstruction rates.
