%0 Journal Article %T 中年人颈动脉WSS与FMD、IMT、心血管危险因素的相关关系<br>Correlation Between Carotid Artery WSS and FMD, IMT and Cardiovascular Risk Factors of Middle-Aged People %A 王文荣 %A 熊开宇 %A 何辉 %J 北京体育大学学报 %D 2018 %R 10.19582/j.cnki.11-3785/g8.2018.03.011 %X 摘要:目的:探讨正常衰老中年人的颈动脉血流剪切力与肱动脉内皮依赖性血管舒张、心血管危险因素之间的相关关系。方法:为北京市昌平区184名中年人提供义务体检,采用问卷调查收集受试者基本信息,排除动脉粥样硬化、冠心病等患重大心血管疾病者;采用美国GE vivid 7彩超仪测试颈动脉内中膜、血流速度、内径并计算血流壁面剪切力;采用血液生化分析仪检测血脂4项;采用血管内皮功能检测仪(日本,UNEX EF)分析肱动脉血流介导的内皮舒张功能。结果:184名受试者中,有男性82人(44.6%)和女性102人(55.4%),肥胖受试者64人(34.7%)。所有受试者腰臀比均超出正常范围,心率、血压、血脂、内中膜均在正常范围内。所有受试者颈动脉壁面剪切力(WSS)与肱动脉血流介导的舒张功能(FMD)之间的关系有显著性差异分别为(r =?0.384,P < 0.01) 。女性颈动脉WSS与FMD之间的关系有显著性差异(r =0.467,P < 0.01)。男性颈动脉WSS与FMD之间的关系(r =0.328,P=0.063)没有显著性差异。所有受试者颈动脉WSS与心血管危险因素均有相关性,颈动脉WSS与IMT(r =-0.254,P <0.05),颈动脉WSS与年龄(r =-0.354,P <0.05),颈动脉WSS与BMI(r =-0.409,P< 0.01),WSS与腰臀比(r =-0.429,P< 0.01)。通过线性回归分析,总样本,颈动脉WSS与颈动脉IMT呈线性相关;女性受试者,颈动脉WSS与颈动脉IMT呈线性相关(b=-0.309,t=-2.647,P=0.011),与BMI呈线性相关(b=-0.392,t=-2.141, P=0.037)。结论:中年人颈动脉WSS与血管内皮功能均在正常范围内;颈动脉WSS与FMD呈正相关,说明伴随血管内皮功能下降颈动脉WSS出现下降;颈动脉WSS与心血管危险因素呈负的线性相关,伴随危险因素的增加逐渐减小,颈动脉WSS可能是血管内皮功能障碍的预测指标之一,具体关系还需要进一步的研究证实。<br>Abstract: Objective: The purpose of this study was to analyze the correlation between carotid artery shear flow and brachial artery endothelium-dependent vasodilatation and cardiovascular risk factors in normal aging middle-aged people. Methods: One hundred and eighty-four middle-aged people in Changping District of Beijing were recruited in this study, and their basic information was collected by questionnaire, and patients with major cardiovascular diseases, such as atherosclerosis, coronary heart disease were excluded. People’s carotid artery intima-media, blood flow velocity and inner diameter were measured by color doppler ultrasound (US GE vivid 7), and the wall shear force was calculated; the serum lipids were tested by blood biochemical analyzer; and the endothelial function of blood flow mediated by brachial artery was analyzed by vascular endothelial function analyzer. Results: In 184 patients, there were 82 men (44.6%) and 102 women (55.4%), and 64 obese subjects (34.7%). The waist to hip ratio of all subjects was beyond the normal range, and heart rate, blood pressure, blood lipid and medial membrane were all within the normal range. There was a correlation between carotid artery WSS and FMD in all people (r = -0.384, P <0.01), and in women (r = 0.467, P <0.01), but not in men (r =0.328, P=0.063). People’s carotid artery WSS was related to cardiovascular risk factors, including IMT (r = -0.254, P <0.05), age (r = -0.354, P <0.05), BMI (r = -0.409, P <0.01) and waist to hip ratio (r = -0.429, P < 0.01).
