%0 Journal Article %T 盐城市亭湖区2型糖尿病患者周围神经病变影响因素调查分析 %A 余文艳 %A 朱丽萍 %J 同济大学学报(医学版) %D 2017 %R 10.16118/j.1008-0392.2017.01.023 %X 目的 调查分析盐城市亭湖区2型糖尿病患者周围神经病变(diabetic peripheral neuropathy, DPN)的影响因素。方法 选择盐城市亭湖区的先锋、毓龙、黄海街道卫生服务中心管理的2型糖尿病患者2946例,采用多伦多临床评分量表(TCSS)展开DPN筛查:0~5分为无DPN,6~8分为轻度DPN,9~11分为中度DPN,12~19分为重度DPN,分析所得的基本资料和实验室数据。结果 DPN阳性率为20.4%(601/2946),其中轻度、中度、重度DPN分别为15.2%(4482946)、4.2%(123/2946)、1.0%(30/2946)。与阴性患者相比,DPN阳性患者性别、FPG、LDL-C、HDL-C、UCR的差异无统计学意义(P>0.05);而年龄、病程、BMI、2hPG、HbA1c、是否合并糖尿病性视网膜病变(diebetic retinitis, DR)及严重程度差异有统计学意义(P<0.05)。Logistic逐步回归分析显示:年龄≥55岁、BMI≥24、HbA1c≥8.5%、合并患有中重度非增殖期DR是该地区2型糖尿病患者DPN筛查阳性的危险因素。结论 盐城市亭湖区2型糖尿病患者的DPN筛查患病率低于既往研究,且以轻度病变为主。年龄≥55岁、BMI≥24、HbA1c≥8.5%、合并患有中重度非增殖期DR是2型糖尿病患者DPN阳性的影响因素。</br>Objective To investigate the risk factors of diabetic peripheral neuropathy (DPN) in patients with type 2 diabetes mellitus (T2DM). Methods Total 2,6 patients with T2DM from the sub-district health service centers in Dinglin District of Yancheng city underwent DPN screening. According to Toronto Clinical Scoring Scale (TCSS), score 0-5 was defined as non-DPN, 6-8 as mild DPN, 9-11 as moderate DPN, and 12-19 as severe DPN. The risk factors of DPN in T2DM patients were analyzed. Results The overall prevalence rate of DPN in T2DM patients was 20.4% (601/2946), and the prevalence rates of mild, moderate and severe DPN were 15.2% (448/2946), 4.2% (123/2946), and 1.0% (30/2946), respectively. There were no significant differences in gender, FPG, LDL-C, HDL-C, and UCR between patients with and without DPN (P>0.05), while there were significant differences in age, course of disease, BMI, 2hPG, HbA1c, comorbidity of diabetic retinitis (DR) and severity of DR between two groups (P<0.05). Logistic stepwise regression analysis indicated that the risk factors of DPN in T2DM patients were age≥55, BMI≥24, and HbA1c≥8.5%, moderate and severe non-proliferative diabetic retinopathy.
