%0 Journal Article %T 经皮冠状动脉介入治疗后高敏肌钙蛋白T升高的相关因素分析 %A 胡铂 %A 王小东 %A 陈旼 %A 罗裕 %A 周华 %A 张奇 %J 同济大学学报(医学版) %D 2018 %R 10.16118/j.1008-0392.2018.01.018 %X 目的 分析经皮冠状动脉介入治疗(percutaneous coronary intervention, PCI)后高敏肌钙蛋白T(high sensitive troponin T, hs-TNT)升高的危险因素。 方法 入选2016年1月至2016年8月因心绞痛接受PCI治疗的107例患者,根据介入治疗后hs-TNT分为两组: hs-TNT正常组(hs-TNT<0.013ng/mL,n=57)、hs-TNT升高组(hs-TNT>0.026ng/mL,n=50)。通过分析患者的临床特点、实验室检查结果及冠脉病变特征及介入情况探讨PCI术后hs-TNT升高的相关因素。结果 两组患者年龄、血糖、Gensini评分、狭窄血管数、干预血管数、支架直径、支架长度、置入支架数、球囊扩张时间、分叉病变和弥漫病变差异有统计学意义(P<0.05)。Logistic多元回归分析提示: 年龄、血糖、置入支架数是PCI术后hs-TNT升高的独立危险因素(P<0.05)。结论 年龄、血糖、置入支架数是经PCI术后hs-TNT升高的独立危险因素。</br>Objective To analyze the risk factors of high-sensitive troponin T(hs-TNT) elevation in patients with coronary artery disease after percutaneous coronary intervention(PCI). Methods One hundred and seven patients with angina pectoris were admitted and underwent PCI in our hospital from January 2016 to August 2016. There were 50 cases with elevated hs-TNT(>0.026ng/mL)and 57 cases with normal hs-TNT(<0.013ng/mL)after PCI. Clinical characteristic, laboratory finding, angiographic and procedural factors were analyzed and compared between two groups. Results There were more patients with older age, higher serum level of glucose, higher Gensini score, higher number of stenosed vessels, higher number of treated vessels, smaller diameter of stents, longer stent length, higher number of stent, higher sums in balloon dilatation druration, higher proportion of bifurcation lesion and diffuse lesion in elevated hs-TNT group than those in normal hs-TNT group (P<0.05). Logistic multivariate analysis showed that older age, higher serum glucose level and more implanted stents were independent risk factors for hs-TNT elevation (P<0.05) following PCI.
