The major problems of mortality in children under five occurred in developing countries with high poverty rates compared with developed countries. General death causes have been identified and are related with perinatal conditions, measles, HIV/AIDS, diarrhea, respiratory diseases and others. Malnutrition has been associated with approximately half of deaths, as the underlying cause in the world. Objective. To describe death rate due with malnutrition with an adjusted method, taking into account the difficulties to identify it as a direct cause of death. Materials and methods. A descriptive study included analysis of the CIE-10 vital statistics from 2003-2007; death rates were estimated too, the method ※analysis of multiple causes§ for infectious diseases was also applied, as well as a calculation of death probabilities. Results. Malnutrition was associated with infectious diseases, the frequency of infectious disease as a direct cause of death was almost seven times higher in cases with the antecedent of malnutrition. Once adjusted the death rate values, it was found the initial value was increased nearly five times. The probability of death after the adjustment for inadequate classification increased about four times. The method "analysis of multiple causes" could establish the effectiveness as a methodology in the setting of this type of deaths in Colombia. Conclusion. Malnutrition maybe a director underlying cause of death in one in eight deaths in children under one year and one in three children one to four years. Introducci車n.
