%X Aim: To determine the incidence of drug-related problems in adult patients with hypertension, that visit community pharmacies, Farmacia Tres R¨ªos and Farmacia D¡äMar¨ªa. Methods: The study was conducted for a period of 12 weeks. Sixty hypertensive patients with al least one prescription for antihypertensive medication were included. The study was carried out with the D¨¢der Method for pharmacotherapeutic follow-up. Results: 135 negative clinical results (NCR) were detected (2.25 NCR/patient); of which 71 problems were due to necessity, 44 were related to effectiveness, and 20 of them were due to security. 135 interventions were carried out by the pharmacist: 104 of these interventions (77.03%) were accepted, being the health problem resolved in an 80.74% of the interventions. Conclusion: The employment of the D¨¢der Method for pharmacotherapeutic follow-up, in hypertension patients of the two community pharmacies of the study, allows the identification of medication negative clinical results and the adequate resolution of the drug-related problem.
