%T Study of prostatic biopsies: indication, performance of biopsy and quality of the anatomical pathology report.

%X Aim: To evaluate the quality of prostatic biopsies in the Centre region on the basis of their indication, their performance and analysis of the anatomical pathology report in comparison with the recommendations of the Agence nationale d'accr¨¦ditation et d'¨¦valuation en sant¨¦ (ANAES - French national agency for healthcare accreditation and evaluation) and the regional oncology network for the Centre region. Methods: Transverse descriptive survey including all prostatic biopsy histology reports compiled by anatomical pathologists from biopsies performed by surgeons in the same region, during the first quarter of 2001. Results: We analysed 606 reports and the deviations observed with respect to the recommendations concerned: - a non-compliant indication for endorectal ultrasound performed for diagnostic purposes before performance of the biopsy in 49 % of cases; - inappropriate biopsy performance conditions, with 64 % of biopsies performed as a hospital inpatient procedure and 47 % under anaesthetic (spinal anaesthetic, neuroleptic analgesia or general anaesthetic); - non-compliance with the quality criteria for the anatomical pathology report, with 44 % of the reports only including one of the four items of clinical information recommended and only 54 % of the reports clearly indicating extension to the peri-prostatic space, a key factor in the therapeutic decision. Conclusion: This study, conducted with a group of experts, demonstrated medical practice deviations with respect to the reference systems. The results have been communicated to healthcare professionals and proposals have been made with respect to improvement of the standard anatomical pathology report and a revision of listing of prostatic biopsy procedures.
