%X Objetivo: describir un nuevo caso de aneurisma cirsoideo gigante de cuero cabelludo. Descripci車n: una mujer de 34 a os de edad present車 una historia de cefaleas y deformaci車n del cuero cabelludo pulsatil que aumentaba de tama o con la maniobra de Valsalva. La resonancia magn谷tica, la angiorresonancia y la angiograf赤a digital mostraron una malformaci車n arteriovenosa de cuero cabelludo alimentada exclusivamente por la arteria car車tida externa derecha. Intervenci車n: se realiz車 primero un clipado transitorio de las principales arterias eferentes para reducir la masa. Luego se hizo un colgajo cut芍neo exponiendo y extirpando la malformaci車n. Se preserv車 la vascularizaci車n del cuero cabelludo. La evoluci車n postoperatoria fue favorable, retrogradando los s赤ntomas. Conclusi車n: existen controversias en el manejo de malformaciones complejas del cuero cabelludo pero, pensamos que la elecci車n de la t谷cnica quir迆rgica fue apropiada para el caso presentado. Objective: to describe a new case of a giant cirsoid aneurysm of the scalp. Description: a woman, 34 years-old, presented a history of headaches and a pulsating scalp deformity that increased in size with the Valsalva manoeuvre. Magnetic resonance, angioresonance and digital angiography showed an arteriovenous malformation of the scalp feeded exclusively by the right external carotid artery. Intervention: We performed first a transient clipping of the main efferent arteries, reducing the flow and the mass. Through a cutaneous flap de malformation was exposed and resected. Scalp vascularisation was preserved. Postoperative outcome was uneventful and the symptoms disappeared. Conclusion: There are many controversies in the management of scalp complex malformations but we think that the surgical technique performed was appropriate for this case.
