%X Una de las razones m芍s frecuentes para instaurar la ventilaci車n mec芍nica es la reducci車n del trabajo respiratorio del paciente, y uno de los problemas m芍s habituales en una unidad de cuidados intensivos es la desadaptaci車n entre el enfermo y el ventilador. Cuando a pesar de que los par芍metros programados en el respirador son te車ricamente los correctos no existe una adecuada sincronizaci車n entre este y el enfermo, estamos ante un paciente que ※lucha§ con el respirador. El objetivo del presente art赤culo es hacer especial hincapi谷 en el diagn車stico adecuado del problema, sin recurrir a la c車moda pero antifisiol車gica medida de exclusivamente medicar a nuestro enfermo, as赤 como no adoptar soluciones globales, sino espec赤ficas para cada tipo de paciente. Para ello clasificaremos las posibles causas de desadaptaci車n en tres grupos: las debidas a desequilibrio en la relaci車n aporte/demanda ventilatorios, las ocasionadas por descoordinaci車n entre la m芍quina y la persona y una vez descartadas 谷stas, las debidas a delirium, dolor, etc. que precisan del tratamiento adecuado. Una vez encuadrado el problema en uno de estos grupos, seguiremos las medidas terap谷uticas adecuadas. Patient Fighting with the Ventilator ABSTRACT One of the most frequent reasons for instituting mechanical ventilation is to decrease a patient＊s work of breathing, and one of the most common problems in an Intensive Care Unit is the maladjustment between the patient and the ventilator. When in spite of the programmed parameters in the ventilator are theorically correct doesn∩t exist a suitable sincronization between it and the patient, we∩re in the presence of a patient who ※struggles§ against the ventilator. The goal of this article is emphasizing in the proper diagnosis of the problem, without using the easy but antiphysiological measure consisting in an exclusive pharmacological treatment, as well as not taking global solutions, but specific ones for every kind of patient. For that reason we∩ll classify the possible causes of maladjustment in three groups: due to imbalance in ventilatory supply/demand ratio, due to discoordination between the machine and the person and once rejected, the ones due to delirium, pain, etc. which need the proper treatment. Once the problem has been classified in one of these groups, we∩ll take the appropriate therapeutic steps.
