%X This report presented a case of in situ intracoronarythrombosis of the proximal left anteriordescending artery (LAD), causing significanttransient myocardial ischemia during electivecoronary angiography in a patient with knowncoronary artery disease (chronic occlusion ofthe circumflex artery, significant stable left mainstenosis) and a severe vasovagal reaction duringfemoral artery puncture. He was treated successfullywith local fibrinolytic therapy, whereasentire diagnostic procedure was completed successfully.There were no cardiac wall motionabnormalities after the procedure, and the rest ofthe hospitalization was uneventful.
