Es una complicaci車n infrecuente del paludismo por Plasmodium falciparum, caracterizada por fiebre, hemoglobinuria y fallo renal, con desarrollo dram芍tico y alta mortalidad. El paciente proven赤a de la Rep迆blica Popular de Angola donde permaneci車 durante 7 a os, con antecedentes de varios episodios de malaria; ingres車 en el Instituto de Medicina Tropical "Pedro Kour赤", con fiebre e 赤ctero y gota gruesa positiva a P. falciparum. Se comenz車 tratamiento con quinina y 12 h despu谷s aparecieron orinas de color rojo vino e incremento del 赤ctero, se retir車 el tratamiento con quinina. Se transfundieron gl車bulos rojos y plaquetas, se realiz車 hemodi芍lisis y plasmaf谷resis. Fue egresado 2 semanas despu谷s asintom芍tico con gota gruesa negativa. Se discuti車 la patogenia de esta complicaci車n, en especial lo referente al fallo renal; se hizo 谷nfasis en la importancia del tratamiento sintom芍tico y de sost谷n, en la evoluci車n del paciente. The first case of blackwater fever was reported in Cuban medical literature. It is an uncommon complication of paludism due to Plasmodium falciparum, characterized by fever, hemoglobinuria, and kidney failure, with dramatic development and high mortality. The patient came from the People*s Republic of Angola, where he stayed 7 years, with history of various episodes of malaria. He was admitted with fever, icterus and thick film test positive for P. falciparum. Treatment with quinine was started, but dark red urine and an increase of icterus were observed 12 hours later. Treatment with quinine was suspended. Red blood cells and platelets were transfused and hemodialysis and plasmapheresis were performed. The patient was discharged 2 weeks later asymptomatic and with negative thick film test.
