%X Se presenta un caso, de un paciente masculino de 34 a os, con ictericia obstructiva y epigastralgia de 3 meses de evoluci¨®n. El examen baritado gastroduodenal demuestra dilataci¨®n del bulbo duodenal y estenosis de la segunda porci¨®n (que impidi¨® colangiopancreatograf¨ªa retr¨®grada endosc¨®pica). En la colecistocolangiograf¨ªa translaparosc¨®pica se comprueba dilataci¨®n del ¨¢rbol biliar extrahep¨¢tico y estenosis del col¨¦doco distal. Durante el acto operatorio se confirma hallazgo de p¨¢ncreas anular y ¨²lceras p¨¦pticas pil¨®ricas; se realiza tratamiento mediante: vaguectom¨ªa, coledocoyeyunostom¨ªa, y gastroyeyunostom¨ªa. La evoluci¨®n ulterior fue satisfactoria. Se revisan aspectos cl¨ªnicos y quir¨²rgicos de inter¨¦s pr¨¢ctico para el diagn¨®stico y el tratamiento de esta embriopat¨ªa infrecuente en el adulto A case of a 34-year-old male with obstructive icterus and epigastralgia of 3 months of evolution was presented. The gastroduodenal examination with barium shows dilatation of the duodenal bulb and stenosis of the second portion (that prevented the endoscopic retrograde cholangiopancreatography). Dilatation of the biliary extrahepatic tree and stenosis of the distal choledocus were confirmed by translaparoscopic cholecystocholangiography. Annular pancreas and pyloric peptic ulcers were found during the operation. The treatment consisted in vagectomy, choledochojejunostomy, and gastrojejunostomy. Clinical and surgical aspects of practical interest for the diagnosis and treatment of this uncommon embryopathia in the adult are reviewed
