%X Se presenta el caso de una paciente enferma de insuficiencia renal cr車nica (IRC), citada para cirug赤a de trasplante renal por sufrir de insuficiencia ventilatoria aguda a consecuencia de gran hematoma arterial como complicaci車n de un cateterismo venoso profundo. Durante la realizaci車n de maniobra de traqueostom赤a de urgencia se produce secci車n total accidental de arteria car車tida derecha, que al momento de la incisi車n quir迆rgica se encontraba en l赤nea media debido al desplazamiento de las estructuras del cuello, por el hematoma en expansi車n. La colocaci車n en la tr芍quea de un cat谷ter de aspiraci車n n迆mero 12, debido a que la intubaci車n traqueal era imposible, permiti車 mantener una adecuada oxigenaci車n durante la colocaci車n de la c芍nula de traqueostom赤a. La adecuada circulaci車n por los vasos arteriales contralaterales permiti車, al parecer, mantener una buena perfusi車n cerebral con lo que al final de la reparaci車n de la secci車n vascular no quedaron en la paciente secuelas neurol車gicas A case of a patient presenting with chronic renal failure admitted to undergo a renal transplantation for suffering an acute respiratory failure resulting from a big arterial hematoma as a complication of a deep venous catheterization, is reported. During the performance of an emergency tracheostomy an accidental total section of the right carotid artery was produced which at the moment of the surgical incision was found to be in the medial line due to a displacement of the neck structures by the expanding hematoma. The placement of a number 12 aspiration catheter because tracheal intubation was not possible, allowed to maintain an adequate oxigenation during the placement of the tracheostomy catheter. The proper circulation through the contralateral arterial vesels allowed to keep a good brain perfusion and at the end of the vascular repair the patient was free of neurologic sequelae
