%X Se presenta el caso de un paciente de 64 a os de edad que acudi車 al cuerpo de guardia con dolor tor芍cico agudo localizado en hemit車rax derecho y disnea intensa. La persistencia del neumot車rax sin que se lograra la reexpansi車n pulmonar con la pleurotom赤a y el drenaje tor芍cico oblig車 a una toracotom赤a. Se encontr車 una bulla enfisematosa gigante rota, y se realiz車 una bilobectom赤a superior y media. El diagn車stico histol車gico fue carcinoma bronquioalveolar. La coincidencia de un c芍ncer de pulm車n en una bulla enfisematosa no es excepcional, pero la aparici車n y persistencia de un neumot車rax como manifestaci車n inicial de un c芍ncer de pulm車n es poco frecuente. This is the case of a patient aged 64 seen in the emergency room due to a acute thorax pain located in right hemithorax and intense dyspnea. The persistence of pneumothorax without to achieve a pulmonary re-expansion with a pleurotomy and thoracic drainage be necessary a thoracotomy. A ruptured giant emphysematous bulla was found thus the superior and middle bilobectomy. The histological diagnosis was a bronchoalveolar carcinoma. Coincidence of a lung cancer in a emphysematous bulla isn't exceptional and the persistence of pneumothorax as initial manifestation of lung cancer is not frequent.
