La invasi車n de otras especialidades en espacios que antes se consideraban exclusivos de los cirujanos generales, el cambio en las exigencias de las EPS que buscan mayor calidad a expensas de especializaci車n en todas las 芍reas, y la estructura actual del sistema que no reconoce el esfuerzo del recurso humano por la obtenci車n de mejores resultados cl赤nicos amenazan la ense anza actual. Para contrarrestar esta tendencia, es necesario procurar la medici車n de los resultados cl赤nicos que permita demostrar que se hacen las cosas bien, hay que enfocar la ense anza en las condiciones que ser芍n abordadas en la pr芍ctica cl赤nica utilizando un ejemplo similar al de la cirug赤a de enfermedades agudas propuesto en Estados Unidos, e implementar el entrenamiento por competencias en lugar de mantener los tiempos fijos de entrenamiento. The education of surgical residents is in critical condition. The takeover by the different specialties that formerly were considered exclusive fields of the general surgeon, the changes and requirements introduced by the insurance corporations in the Colombian health system that seek greater quality at the expense of specialization in all areas, and the current structure of the health system that does not recognize the efforts by the health personnel to achieve better clinical results, are all factors threatening the education of general surgeons. In order to respond to this trend, it becomes necessary to measure clinical results that allow the demonstration that currently things are being well done, and thus focus education on the conditions that will be approached in clinical practice, utilizing a an example the recommendations on general surgery that are being posted in the United States of America.
