El esfigmoman車metro por su f芍cil manejo y transportaci車n y adem芍s, por incruento y preciso, comenz車 a emplearse en la exploraci車n cl赤nica rutinaria. Esto llev車 a que se cayera en el facilismo de considerar la HTA como una serie de cifras escalonadas. A迆n disminuyendo con tratamiento las cifras a niveles considerados normales, el riesgo total de morbilidad y mortalidad cardiovascular sigue siendo aproximadamente de 75 %. La mortalidad por cardiopat赤a hipertensiva comenz車 a disminuir de manera significativa cuando apenas se contaba con medicamentos antihipertensivos, lo que lleva a pensar que los otros componentes del s赤ndrome comienzan a diagnosticarse separadamente y dejan sola la hipertensi車n que no es causa de un riesgo mayor de 25 %. Tampoco pueden considerarse las cifras altas de presi車n arterial como un factor de riesgo cl芍sico, ya que no precede, ni es causa de la manifestaci車n principal del s赤ndrome: la aterosclerosis, que adem芍s, se manifiesta desde etapas fetales o en la primera infancia cuando no hay HTA o si la hubiese, no habr赤a tenido tiempo de actuar. Siguiendo las evidencias expuestas, concluimos que la HTA, especialmente la presi車n sist車lica, es un indicador de riesgo subrogado del s赤ndrome aterotromb車tico. Este s赤ndrome se debe a m迆ltiples causas o factores de riesgo que ocasionan cambios funcionales y/o morfol車gicos de las arterias. Se presentan los indicadores del s赤ndrome que est芍n en uso cl赤nico y se propone una definici車n de la HTA como parte del s赤ndrome aterotromb車tico. This review was made aimed at justifying a new concept of arterial hypertension. Due to its easy use and transportation and to its accuracy, the sphygmomanometer began to be utilized in the routine clinical examination, which led to consider arterial hypertension as a series of stepped figures. Even when these figures are reduced to normal levels by treatment, the total risk for cardiovascular mortality and morbidity is still of approximately 75 %. The mortality from hypertensive heart disease started to decrease significantly when there were just a few hypertensive drugs. This makes us think that the other components of the syndrome began to be separately diagnosed, leaving alone hypertension, which is not the cause of a risk higher than 25 %.
