Combination and transformation of toxin and blood stasis in etiopatho-genesis of thrombotic cerebrocardiovascular diseases

Abstract: : According to the basic theory of traditional Chinese medicine (TCM), the pathogenetic factors such as platelet activation, adhesion, congregation and thrombosis fall into the category of blood stasis, while the pathological changes such as tissue necrosis, oxidative stress injury and inflammation, etc, are far beyond the etiological category of blood stasis. The toxin or the combination and transformation of toxin and blood stasis of TCM are involved in the pathogenesis of thrombotic cerebrocardiovascualr diseases. It is significant to recognize and stress the combination and transformation of toxin and stasis in pathogenicity so as to enrich TCM etiology and improve TCM clinical efficacy in the treatment of cerebrocardiovascular and thrombotic diseases.
Research on quantified diagnosis and combining diseases with syndrome of blood stasis

Abstract: Objective: To explore the way of quantified diagnosis of blood stasis syndrome (BSS) and the essence of BSS. Methods: Using t-test and logistic regression to analyze the symptoms, signs, and objective indexes of BSS in clinical test. Results: (1) The levels of endothelin, nitric oxide, and t-PA between 182 patients with BSS and non-BSS had notable difference. (2) The stepwise regression analysis about hemoglobin, triglyceride, total cholesterol, endothelin, nitric oxide, and t-PA in 170 patients with BSS or non-BSS revealed that the order of these indexes considering their importance was: endothelin, hemoglobin, t-PA, nitric oxide. However, the triglyceride and total cholesterol could not enter the regression equations. (3) The stepwise regression analysis about 40 symptoms and signs in 601 patients with BSS or non-BSS revealed that the 18 items had the very contribution to diagnose the BSS when the F value was 6, and so a regression equation was available. The total coincidence ratio was 94.24% when the other quantified data about BSS were tested in the equation. Conclusion: The standardized TCM syndrome, which accorded with the criteria of combining diseases with syndrome,was made up of symptoms, signs, and objective indexes.The standard could be obtained by multi-center, prospective, random and controlled clinical epidemiological survey and clinical test.
Duplication of an animal model of myocardial ischemia with blood stasis syndrome in mini-swines

Abstract: Objective: To establish an animal model of myocardial ischemia with blood stasis syndrome in mini-swines.Methods: An animal model of myocardial ischemia was established in mini-swines by oppressing the coronary artery through the expansion of inner layer of Ameroid constrictor and the Ameroid constrictor was implanted into the distal end of the initial part of the first branch of interventricular septum of ramus descendens anterior arteriae coronariae sinistrae. Dynamic observation of behavior changes, general health status and changes of hemorheological parameters in the mini-swines were made after operation.Results: The coronary angiography showed that the stenosis rate in ischemic group was more than 75% four weeks after operation. Compared with before operation and sham-operated group, there were great changes of behavior, general health status, tongue color and hemorheological parameters in ischemic group (P<0.05).Conclusion: The animal model of myocardial ischemia with blood stasis syndrome in mini-swines was established successfully 4 weeks after operation. The pathological process in the animal model is similar to that in the patients with chronic myocardial ischemia. So this model can be adopted in the research of myocardial ischemia with blood stasis syndrome.
Evaluation of tongue manifestation of blood stasis syndrome and its relationship with blood rheological disorder in a rat model of transient brain ischemia

Abstract: Objective: To study the tongue tissue blood oxygen saturation measurement for evaluating tongue manifestation of blood stasis syndrome, and to explore its correlation with blood rheological disorder in a rat model of acute transient brain ischemia.Methods: Twentyeight SD rats were randomly divided into shamoperated group and ischemia group. Middle cerebral artery occlusion was induced by thread in rats of the ischemia group. Tongue tissue blood oxygen saturation, neurological severity score and the changes of blood viscosity, red blood cell deformity, thrombin time and fibrinogen in the rats were measured after 24hour reperfusion.Results: Blood viscosity and the content of fibrinogen in the ischemia group were significantly higher than those in the shamoperated group. Red blood cell deformity, thrombin time and tongue tissue blood oxygen saturation in the ischemia group were decreased as compared with the shamoperated group. There was a positive correlation between red blood cell deformity and tongue tissue blood oxygen saturation.Conclusion: Tongue tissue blood oxygen saturation is a good measurement for evaluating blood stasis in a rat model of focal cerebral ischemia, and this model can be used as a rat model of stroke with blood stasis syndrome.
Correlation between blood stasis syndrome and pathological characteristics of coronary artery in patients with coronary heart disease

Abstract: Objective: To study the correlation of blood stasis syndrome or its accompanied syndromes with Gensini score in patients with coronary heart disease (CHD) in stable condition.Methods: The syndrome types of traditional Chinese medicine (TCM) and blood stasis score in 131 CHD patients confirmed by coronary angiography were recorded. Gensini score was calculated according to the coronary pathological characteristics showed by angiography. The correlations of blood stasis syndrome and its accompanied syndromes with coronary lesion and Gensini score were analyzed.Results: Among the TCM syndrome types, blood stasis, turbid phlegm and qi deficiency were the most common syndromes, revealed in 85 patients (64.9%), 83 patients (63.4%) and 85 patients (64.9%), respectively. The coronary lesion length and Gensini score in the patients with blood stasis syndrome were much higher than those in the patients with non-blood stasis syndrome (P<0.05 or P<0.01). In the subtypes of blood stasis, the coronary lesion length and Gensini score in the patients with blood stasis accompanied by turbid phlegm syndrome were higher than those in the patients with non-blood stasis syndrome (P<0.05). And in the patients whose blood stasis syndrome score was more than 9 points, the coronary lesion length was higher than that in the patients whose blood stasis syndrome score was less than 9 points (P<0.05). Besides, with bivariate analysis, the blood stasis syndrome score showed no correlation with Gensini score (Pearson correlation coefficient was 0.104, P=0.241).Conclusion: Blood stasis syndrome is the most common TCM syndrome in CHD patients in stable condition. The blood stasis syndrome score is proportional to coronary lesion length, and reflects the severity of coronary lesion.
Changes of eye signs in chronic hepatitis B patients with blood stasis syndrome

Abstract: Objective: To study the characteristics of eye signs in chronic hepatitis B patients with blood stasis syndrome and the association between blood stasis syndrome and eye signs. Methods: A total of 126 patients with chronic hepatitis B were randomly divided into four groups: non-blood stasis group, and mild, moderate and severe blood stasis groups. The changes of eye signs in the four groups were tested and compared. Results: There were significant differences in scores of blood stasis and eye signs among the four groups (P<0.05 or P<0.01). The changes of eye signs in chronic hepatitis B patients with blood stasis syndrome were different with those with non-blood stasis syndrome. There were no significant differences in each item of the eye signs among the four groups (P>0.05).Conclusion: The eye signs of blood stasis syndrome are applicable, objective and convenient for diagnosing blood stasis syndrome in patients with chronic hepatitis B.

Abstract: Metabonomics analysis of the urine of rats with Qi deficiency and blood stasis syndrome has been performed by comparison with those of normal rats based on NMR techniques. The relative contents of formate, creatinine, 2-oxoglutarate (2-OG), citrate, taurine, trimethylamine-N-oxide(TMAO), succinate and hippurate in the urine of the rats with Qi deficiency and blood stasis syndrome have been changed. These results have provided evidence for understanding the mechanism and the therapy of Qi deficiency and blood stasis syndrome.
Investigation of gene expression profiles in patients with blood stasis syndrome

Abstract: Objective: To investigate the differential gene expression profiles in patients with blood stasis syndrome by oligonucleotide microarray technique.Methods: Sixteen patients with blood stasis syndrome were divided into patients with coronary heart disease (CAD) (n=8) and non-CAD patients (n=8) by using coronary angiography. The sex- and age-matched eight healthy persons were enrolled as control group. Venous bloods were collected for extracting RNA. Test-3 chip was first employed to examine the quality of samples. Then the samples were hybridized with Affymetrix U133 Plus 2.0 array to compare the gene expression profiles among the three groups. Gene-array scanner and gene chip operating software were applied to screen hybridization signals and analyze gene expression respectively. Based on the comparison of the three groups of samples, the differential genes related with blood stasis syndrome were analyzed by Gene Ontology (GO) and pathway, and confirmed by real-time reverse transcription polymerase chain reaction (RT-PCR).Results: Forty-eight differential genes were found being associated with blood stasis syndrome, including 26 up-regulated genes and 22 down-regulated genes. Five of the forty-eight genes (10.4%) were related to inflammatory reaction and immune response through the GO analysis. In the pathway analysis, five of ten significant pathways were referred to inflammation and immune response. The results of real-time RT-PCR proved the accuracy of the gene chip.Conclusion: Inflammatory- and immune-related genes have a remarkable predominance in blood stasis syndrome gene expression profiles, which may explain the function of inflammation and immune response in the occurrence and development of blood stasis syndrome.
Diagnostic value of assay of brain natriuretic peptide in patients with sepsis of blood-stasis syndrome

Abstract: Objective: To explore the diagnostic value of brain natriuretic peptide (BNP) level in patients with sepsis of blood-stasis syndrome.Methods: The prospective method of clinical diagnostic test and evaluation principles of diagnostic test were applied. One hundred and seventy-four patients with sepsis were divided into two groups: blood-stasis syndrome group and non blood-stasis syndrome group. The levels of serum BNP in two groups were detected.Results: The level of BNP in sepsis patients was related to blood-stasis syndrome (P<0.01). BNP cut-off level was 150 ng/L, sensitivity was 85.3%, specificity was 81%, the maximum value of Youden index was 0.663, and the area under the receiver operating characteristic curve (ROC) value was (0.886±0.025).Conclusion: BNP can be used as an objective index of blood-stasis syndrome diagnosis for sepsis, and BNP with boundary value of 150 ng/L is an optimal biological index.
