Las invisibles: una lectura desde la salud pública sobre la violencia sexual contra ni?as y mujeres Colombianas en la actualidad

Abstract: objective: this article is intended to expose the present situation of sexual violence in colombia from the perspective of a gender-based public health approach. methods: identifying, compiling and t he documentary analysis of a set of studies carried out in colombia 1993-2006 and other sources concerning sexual violence and public health in this country. the studies were selected through reviewing electronic databases and library catalogues. results: this article displays some concepts about sexual violence and women’s rights; it characterises the problem amongst colombian children and women. it discusses current public policy in colombia regarding this issue and targets some critical areas for future action. conclusions: the above is aimed at encouraging ref lection, investigation, analysis and inter- disciplinary, effective and sensitive action regarding related gender matters. although this issue is of the utmost importance, it has lagged behind in the midst of the complex panorama of public health in colombia.

Abstract: Objetivo: este artículo pretende exponer la situación actual de la violencia sexual en Colombia, desde la perspectiva de una salud pública con enfoque de género. Metodología: identificación, recopilación y análisis documental de un conjunto de estudios y otras fuentes sobre violencia sexual y salud pública en Colombia correspondientes al período 1993 -2006, seleccionadas mediante revisión de bases de datos electrónicas y catálogos de bibliotecas. Resultados: se presentan algunos conceptos sobre la violencia sexual y los derechos humanos de las mujeres; se caracteriza la magnitud del problema entre las ni as y mujeres de nuestro país; se discuten algunas políticas públicas desplegadas en Colombia ante el tema; y se se alan áreas críticas para la acción futura. Conclusiones: lo encontrado conduce a considerar que es necesario estimular la reflexión, la investigación, el análisis y las acciones interdisciplinarias efectivas y sensibles a los asuntos de género en torno a este tema, que no obstante su importancia ha permanecido relegado en medio del complejo panorama de la salud pública nacional. Abstract: through the presentation of a case of sexual violence to a minor, the weaknesses of the actual health system are exposed with regard to the support services to violence victims that can potentially develop affective disorders secondary to their experiences. it also illustrates the resources that society has developed, along with the successes it has had in facing the limitations imposed by the system. as part of the development of the topic, a portion of the article is dedicated to the treatment of mental disorders as part of health altera-tions known under the name of pro-fessional illness. these topics intend to invite reflection on and redefine the risk that professionals who work in the mental health area are exposed to every day.
Reflexiones en torno al análisis de la situación de salud mental en Colombia, 1974-2004*

Abstract: background: this article explores various concepts of mental health in colombian official studies spanning 30 years. objective: to explore the methods used for conceptualizing mental health and the health situation in a set of studies carried out in colombia between 1974- 2004. methods: identification, recompilation, and analysis of documents of primary sources, in light of the categories of study selected through the previous review of secondary sources following the parameters of historiographical analysis. outcomes: the concept of mental health as the absence of mental disease is the prevailing concept, as well as the characterization of the situation of mental health/disease from out-of-context epidemiological perspectives, without any continuity among the different studies in relation to the methodology or analytical categories. conclusions: it is necessary to consider problems in the field of mental health from a perspective linking the current times to historical processes, allowing for the exposure of their configuration, as well as for the planning of studies that permit the analysis of trends.

Abstract: A través de la presentación de un caso de violencia sexual en una menor, se presenta la debilidad del actual sistema de salud respecto a los servicios de apoyo a las víctimas de violencia, quienes potencialmente pueden desarrollan trastornos afectivos secundarios a sus experiencias. También ilustra los recursos exitosos que la sociedad ha desarrollado para enfrentar las limitaciones impuestas por el sistema. En el desarrollo del tema, una sección del artículo está dedicada al tratamiento de los trastornos mentales, como parte de las alteraciones de salud conocidas con el nombre de enfermedad profesional. Estas líneas pretenden ser una invitación a reflexionar y a redefinir el riesgo al que están expuestos en su vida diaria los profesionales que trabajan en el área de la salud mental. Through the presentation of a case of sexual violence to a minor, the weaknesses of the actual health system are exposed with regard to the support services to violence victims that can potentially develop affective disorders secondary to their experiences. Abstract: Introducción: el artículo explora el modo en que la salud mental se ha concebido como objeto de estudio en algunos análisis oficiales colombianos sobre la situación nacional de salud, a lo largo de un período de treinta a os. Objetivo: explorar cómo se han operado los conceptos de salud mental y situación de salud en un conjunto de estudios realizados en Colombia entre 1974-2004. Método: identificación, recopilación y análisis documental de fuentes primarias, a la luz de categorías de estudio seleccionadas mediante la revisión previa de fuentes secundarias, siguiendo los parámetros del análisis historiográfico. Resultados: predomina la concepción de salud mental como ausencia de enfermedad, así como la caracterización de su situación desde perspectivas epidemiológicas descontextualizadas, sin que exista continuidad entre trabajos en cuanto a metodologías o categorías analíticas. Conclusiones: es necesario estudiar la problemática de la salud mental desde una perspectiva articuladora del momento con los procesos históricos que le permitieron su configuración, así como planear estudios que permitan el análisis de tendencias. Aspectos psicopatológicos del coeficiente intelectual limítrofe: un estudio en el Hospital de la Misericordia, 2000-2005

Abstract: introduction: poor academic functioning is a common chief complaint in children and adolescent psychiatry services; its possible causes could be attention- defi cit/hyperactivity disorder (adhd), emotional disorder in childhood, learning disorders, sensorial defi cits and mental retardation. but there is an abnormal condition without a specifi c nosological classifi cation: borderline intellectual functioning (bif), determined when iq is between 71 and 84. objectives: to describe the sociodemographic characteristics and comorbilities in patients with bif evaluated by the children and adolescent psychiatry service al la misericordia hospital from 2000 to 2005. methods: this is a descriptive study. we reviewed the data base and medical histories of patients with wisc r between 71 and 84. results: 161 patients were found. 68.94% (n=111) were men; the mean age was 11.5 years old. 92.53% were poor. the most common chief complaint was poor academic functioning 40.37% (n=65); a history of psychomotor delay 20.49% (n=33) and school failure 19.87% (n=32) was common. adhd 57.14% (n=92) and depression and anxiety 22.98% (n=37) were the most common comorbilities. conclusion: bif with unfavorable socioeconomic conditions could increase the individual’s vulnerability to psychopathology and compromise the quality of life.
Aproximación narrativa a las intervenciones en salud para mujeres y ni?as supervivientes de violencia sexual en la Red Suroccidental de Bogotá, 2003-2004

Abstract: introduction: the purpose of this study is to describe the characteristics of health care offered to victims of sexual violence in the southwestern of bogotá, colombia. the characterization is made from the perspective of those directly involved, based on their description of their individual experiences, the quality of the services received and their recommendations for the improvement of these services. method: we employed narrative and paradigmatic analysis of interviews and group meetings. sampling was structural and not probabilistic. it included 11 subjects (4 patients, 4therapists, 2 community mental health providers and 1 victim’s family member) from health care institutions in the district of fontibón, bosa, kennedy and puente aranda, in bogotá. results: our analysis suggests two positive aspects: the presence of a government-based citywide policy on this subject and the humane qualities of the healthcare workers involved. negative aspects include the lack of mental health coverage, access barriers, discontinuity of treatment, lack of intervention by social networks and defi cient early detection of cases. conclusions: the individuals exhibited a degree of conceptual richness in the topic of sexual violence that could be used in the process of planning policies and services in this area. interventions in sexual violence should be considered urgent and administrative barriers between victims and interventions should be overcome.

Abstract: Introducción: El bajo rendimiento académico es un motivo de consulta frecuente en psiquiatría infantil, con posibles causas como el trastorno por défi cit de atención con hiperactividad (TDAH), los trastornos emocionales de la infancia, los trastornos del aprendizaje, las defi - ciencias sensoriales y el défi cit cognoscitivo. Sin embargo, existe una condición anómala que sin pertenecer a una clasifi cación nosológica específi ca, toma cada vez más importancia en la práctica clínica: el coefi ciente intelectual limítrofe (CIL), defi nido por el puntaje entre 71 y 84. Objetivos: Describir las características sociodemográfi cas y las comorbilidades más frecuentes de pacientes con CIL en el servicio de Psiquiatría Infantil del Hospital La Misericordia entre los a os 2000 y 2005. Materiales y métodos: Estudio descriptivo. Revisión de base de datos e historias clínicas de pacientes con WISC R entre 71 y 84. Resultados: Se recolectaron datos de 161 pacientes: 68,94% (n=111) fueron varones; la edad promedio fue 11,5 a os; el 92,53% pertenecía a estrato socioeconómico bajo y el motivo de consulta más frecuente fue bajo rendimiento académico en 40,37% (n=65); el antecedente personal más frecuente fue retardo psicomotor, en 20,49% (n=33); 19,87% (n=32) repetía a o escolar. El TDAH fue la comorbilidad más frecuente, con 57,14% (n=92), seguido por depresión y/o ansiedad, con 22,98%(n=37). Conclusión: El CIL, sumado a condiciones socioeconómicas desfavorables, es un factor que puede aumentar la vulnerabilidad del individuo a cualquier psicopatología y afectar su calidad de vida. Abstract: Introducción: Esta investigación buscó dar cuenta de las características de la atención en salud a las víctimas de violencia sexual en la Red Suroccidental de Bogotá desde la perspectiva de los protagonistas, develando sus narrativas, la atención recibida y las recomendaciones para mejorar tales servicios. Métodos: Investigación social de segundo orden, con análisis narrativo y paradigmático de entrevistas en profundidad y equipos refl exivos. La muestra fue estructural y no probabilística, de 11 personas (4 pacientes, 4 terapeutas, 2 referentes de salud mental comunitaria y 1 familiar de la víctima) pertenecientes a los servicios de atención en salud de Fontibón, Bosa, Kennedy y Puente Aranda, en Bogotá. Resultados: Los lineamientos de política distrital en el tema y la calidad humana de los trabajadores de la salud involucrados son las principales fortalezas. Como aspectos negativos, la defi ciente cobertura de atención en salud mental, barreras de acceso, abandono del tratamiento, cobro por la prestación de servicios, poca intervención de redes sociales, defi ciente captación temprana del caso. Conclusiones: La riqueza conceptual sobre el tema exhibida por los entrevistados debería aprovecharse en planeación participativa de políticas y servicios sanitarios. Debe prevalecer el carácter urgente e integral de estas intervenciones sobre cualquier barrera administrativa entre la usuaria y el restablecimiento de sus derechos en salud.
