Idiopathic Interstitial Pneumonias (IIPs): Review of Clinical, Radiographic and High-Resolution Computed Tomography (HRCT)  [PDF]

Abstract: Making a confident diagnosis is a complex task for a specific form of interstitial lung disease and providing appropriate management in an attempt to achieve normalization of the disease can put up an alarming process for the clinicians. A set of diffuse and restrictive lung diseases incorporate with idiopathic interstitial pneumonias, showing inflammation and fibrosis of the interstitium due to parenchymal damage. High-resolution computed tomography (HRCT) has magnified the diagnostic standpoint in stepwise identification and classified various patterns in the evaluation of interstitial lung disease. The aim of our review is to elaborate clinical, radiographic and typical and atypical HRCT findings of idiopathic interstitial pneumonias by correlating with its differential diagnosis. Idiopathic pulmonary fibrosis is the most predominant idiopathic interstitial pneumonias and its diagnosis needs to omit all other well-known causes of interstitial lung diseases. According to the 2011 evidence-based guidelines, usual interstitial pneumonia can be diagnosed by HRCT when all criteria are fulfilled. Non-specific interstitial pneumonia is distinguished by bilateral patchy ground-glass opacities and irregular linear/reticular opacities. Respiratory bronchiolitis associated-interstitial lung disease and desquamative interstitial pneumonia show centrolobular nodules and ground-glass opacities as imaging patterns. Cryptogenic organizing pneumonia consists of patchy peripheral or peribronchial consolidations, while ground-glass opacities with tendency for migration, which is evolving to fibrosis, in acute interstitial pneumonia. Lymphoid interstitial pneumonia and idiopathic pleuro-parenchymal fibroelastosis are classified under rare idiopathic interstitial pneumonias. HRCT images help radiologists in diagnosis and mapping specific patterns of idiopathic interstitial pneumonias. This article reviews the stages of evolution in HRCT features for idiopathic interstitial pneumonias.

Abstract: We are reporting a case of a 51-year-old male patient with 3-year history of Rheumatoid Arthritis who developed gynecomastia 2 - 3 months after starting Methotrexate (MTX) therapy, without folate supplementation. Two months after stopping MTX therapy and initiating folate supplementation, gynecomastia started resolving. Very few cases of gynecomastia due to MTX therapy have been reported worldwide, although it is a rare yet a significant occurrence and should always be considered in male patients with Rheumatoid Arthritis.
Low Concentration of Silver Nanoparticles Not Only Enhances the Activity of Horseradish Peroxidase but Alter the Structure Also

Abstract: Chemical synthesis of Ag-NPs was carried out using reduction method. The reduction mechanistic approach of silver ions was found to be a basic clue for the formation of the Ag-NPs. The nanoparticles were characterized by UV-vis, FT-IR and TEM analysis. We had designed some experiments in support of our hypothesis, “low concentrations of novel nanoparticles (silver and gold) increases the activity of plant peroxidases and alter their structure also”, we had used Ag-NPs and HRP as models. The immobilization/interaction experiment had demonstrated the specific concentration range of the Ag-NPs and within this range, an increase in HRP activity was reported. At 0.08 mM concentration of Ag-NPs, 50% increase in the activity yield was found. The U.V-vis spectra had demonstrated the increase in the absorbance of HRP within the reported concentration range (0.06–0.12 mM). Above and below this concentration range there was a decrease in the activity of HRP. The results that we had found from the fluorescence spectra were also in favor of our hypothesis. There was a maximum increase in ellipticity and α-helix contents in the presence of 0.08 mM concentration of Ag-NPs, demonstrated by circular dichroism (CD) spectra. Finally, incubation of a plant peroxidase, HRP with Ag-NPs, within the reported concentration range not only enhances the activity but also alter the structure.

Abstract: The gravitational instability of a thermally conducting self-gravitating system permeated by a uniform and oblique magnetic field has been analyzed in the framework of Tsallis’ nonextensive theory for possible mod-ifications in the Jeans’ instability criterion. It is concluded that the instability criterion is indeed modified into one that depends explicitly on the nonextensive parameter. The influence of thermal conductivity on the system stability is also examined.

Abstract: Assam like in other parts of India followed the law given by Manu. Manu established a patriarchal system and women were given a position of subordination and subjugation. Women were taught to be feminist and feminine woman do not want higher education, career and independence, Those who did were frowned upon by not only males but by their own sex as well. All they had to do was to devote their lives from early girlhood to find a husband and bearing children for him. There was no way she could even dream about herself except as her children's mother or her husband's wife.The women did not even have the freedom to choose their husbands. After their marriage they were advised on how to keep their husbands happy at home and it was generally considered their fault if the men turned elsewhere for their pleasure.
The Correlation between X-ray spectral slope and FeKalpha line energy in radio-quiet active galactic nuclei

Abstract: A significant correlation between FeKalpha line energy and X-ray spectral slope has been discovered among radio-quiet active galactic nuclei. The ionization stage of the bulk of the FeKalpha emitting material is not the same in all active galactic nuclei and is related to the shape of the X-ray continua. Active galactic nuclei with a steep X-ray spectrum tend to have a fluorescence FeKalpha line from highly ionized material. In the narrow-line Seyfert 1 galaxies with steeper X-ray spectrum (Gamma_X > 2.1), the FeKalpha line originates from highly ionized material. In the Seyfert 1 galaxies and quasars with flatter X-ray spectrum (Gamma_X < 2.1), bulk of the FeKalpha emission arises from near neutral or weakly ionized material. The correlation is an important observational characteristic related to the accretion process in radio quiet active galactic nuclei and is driven by a fundamental physical parameter which is likely to be the accretion rate relative to the Eddington rate.

Unruptured Middle Cerebral Artery Aneurysm in a Patient with Systemic Lupus Erthematosus: A Diagnostic Challenge

Abstract: A 22-year-old female patient presented to the Emergency Department of a tertiary care hospital with symptoms of headache and nausea. She has been on a regular follow-up for the preceding three and a half years after being diagnosed as systemic lupus erythematosus (SLE). She had been treated earlier for SLE nephritis in the same institution, and had two relapses of nephrotic syndrome in the last three and a half years for which she had been treated and had achieved complete remission. All possibilities of headaches in background of SLE were considered. CNS examination was inconclusive. There was no nuchal rigidity or no cranial nerve deficits. Fundoscopy and Plain CT scan of brain were normal. The possibility of CNS-lupus was considered considering the high values of antiphospholipid antibodies (APLA). Treatment was initiated accordingly; however, there was no improvement in her symptoms. Although being rare in a patient with SLE, the possibility of an aneurysm was considered. Four vessel digital substraction angiography revealed two unruptured aneurysms of 7.2 mm and 3.9 mm in the left middle cerebral artery (MCA) territory. Craniotomy and aneurysmal clipping was done successfully, and the patient was relieved of her symptoms. A high degree of suspicion towards a rarer cause clinched the diagnosis of a left MCA territory stem artery aneurysm. This rationale of strong suspicion and discussion of differential diagnosis brought a change in the management of the patient.
