Abstract: the purpose of this research was to assess the relation between mothers’ guidance speech and children’s participation in the problem-solving tasks. fifty mother-child dyads were videotaped during two joint problem-solving tasks. the children were aged between 3 and 5. the tasks were designed to elicit a didactic setting (the construction of a series of pieces of different lengths, with and without a model). the mothers’ speech was analysed according to its content, as per wertsch’s (1985) view of the semiotic devices that adults use to establish inter-psychological functioning (abbreviation and referential perspective), and considering metacognitive and cognitive mediation. the children’s participation was assessed on the basis of the extent to which they were responsible for solving the problem and was related to the mothers’ scaffolding speech, with consideration given to the variables mother’s formal schooling and the children’s cognitive and language development. the results reveal that the mothers adopted a general guidance speech script, but in the seriation with a model the metacognitive challenges were more frequent, while in the series without a model it was the cognitive mediation that was used relatively more often. however, there was no significant relationship between the discourse of the mothers who employed higher level metacognitive or cognitive challenges and the children’s participation in the tasks. these results are discussed in terms of the child and mother variables on the basis of the socio-constructivist perspective taken by karmiloff smith’s representational redescription model (1992).

Abstract: this paper approaches the role of sign language in the construction of deaf identity. various authors have discussed how language relates to the construction of identity, pointing out that identity constitutes through meaning - when a subject means they becomes meaningful (orlandi, 1998). we thus attempt to link this discussionto the field of deaf studies, considering that in the case of deaf children the privileged interaction partner is another deaf person. most students have their first contact with this language in schools and institutions for the deaf. we have observed advantages when deaf teachers take over classroom teaching: one is that students are able to develop narrative constructions in sign language; another one is that this experience enables them to perceive themselves as deaf, and construct a deaf identity as early as 5-7 years., when they take on and differentiate roles in interaction, especially with regard to the deaf teacher and the hearing teacher. in the field of deafness, the bilingual education approach anticipates deaf people's awareness of the meaning of deafness, which until quite recently was occurred in adulthood.

Abstract: A 4-year-old boy was evaluated for severe hypercholesterolemia (cholesterol: 831 mg/dL) and disseminated xanthomas. Both parents had hypercholesterolemia: mother (cholesterol: 308 mg/dL) and father (cholesterol: 281 mg/dL). There was no family history of cardiovascular disease. Skin examination showed: intertriginous xanthomas of feet and hands, tuberous xanthomas in knees and elbows, tendinous xanthomas in Achilles tendon and xanthomas in the gluteal region, associated with corneal arc bilaterally. Laboratory work-up excluded secondary causes of hypercholesterolemia and a diagnosis of homozygous familial hypercholesterolemia was made. Echocardiogram showed bicuspid aortic valve and mild aortic insufficiency. Doppler ultrasound of carotid arteries and computerized tomography of the thorax for assessment of calcium scoring were normal. The patient’s serum lipids were reduced by approximately 40% after a diet, atorvastatin and ezetimibe. Homozygous familial hypercholesterolemia is an important risk factor for atherosclerosis and premature coronary artery disease in children and young adults. Early diagnosis and treatment with screening of first-degree relatives is essential to minimize the progression of cardiovascular disease in these patients.

Abstract: Analisa-se, por meio desse estudo, o conflito que há entre o direito à vida com outro direito ou princípio fundamental: o direito à recusa ao tratamento vital, baseado na liberdade religiosa, dignidade da pessoa humana e autonomia da vontade do paciente. Efetua-se um estudo mais específico acerca da recusa de tratamento vital em confronto com a vida, com a apresenta o de possíveis solu es para o problema. A recusa a tratamentos médicos ou interven es cirúrgicas, embora bastante discutida na doutrina, deve ser respeitada, salvo se colidir com a vida, hermeneuticamente considerada como um bem maior. Nesse contexto, constata-se também que nenhum progresso científico pode ser obtido com a destrui o da vida e da dignidade humana.
