Abstract: The paper is based on a primary survey of 1095 patients from the Medicine, General Surgery and Cardiac departments in R.G. Kar Medical College and Hospital (a major public sector health care institution in the metropolitan city of Kolkata, India). The results show that public health care institutions remain a vital life support system of the poorer sections of the population. However, this is not a matter of choice, but necessity. The breakdown of the three tiered referral system in the region has resulted in the Hospital, originally conceived as a super specialty hospital and referral health care institution for patients from the adjacent district of North 24 Parganas, functioning as a diagnosis unit. The resultant pressure has exceeded the carrying capacity of the institution and led to poor quality of health care in the Hospital. Revitalizing lower level institutions is therefore essential not only to increase accessibility of health services, but also to ensure efficiency in higher level health care institutions.

Abstract: In many countries information on expectations collected through consumer confidence surveys are used in macroeconomic policy formulation. Unfortunately, before doing so, the consistency of responses is often not taken into account, leading to biases creeping in and affecting the reliability of the indices hence created. This paper describes how latent class analysis may be used to check the consistency of responses and ensure a parsimonious questionnaire. In particular, we examine how temporal changes may be incorporated into the model. Our methodology is illustrated using three rounds of Consumer Confidence Survey (CCS) conducted by Reserve Bank of India (RBI).

Abstract: This paper presents discontinuous space vector PWM (DPWM) techniques for a seven-phase voltage source inverter (VSI). Space vector model of a seven-phase VSI shows that there exist 128 space vectors with different lengths and maps into fourteen sided polygons. A number of possibilities could arise to implement modulation of inverter legs due to large number of available space voltage vectors. Two strategies are adopted here; one utilising large and two middle sets of space vectors to implement discontinuous space vector PWM. Clamping of legs of inverter to either positive or negative dc bus leads to discontinuity in the switching and consequently offers reduced switching loss modulation strategy. A significant reduction in switching losses can be achieved while employing DPWM in a seven-phase VSI. A generalised method is also proposed to realize the DPWM in a seven-phase VSI. Comparison of continuous and discontinuous PWM is presented in terms of switching current ripple. The experimental set-up is illustrated and the experimental results are presented.

Abstract: Power transformers are interface between different voltage levels of essential importance. Because of the long manufacturing processes, transformers are one of the most critical and expensive equipment and so this is one of the reasons why condition monitoring becomes more popular. Monitoring systems as basis for diagnostics open the possibility for expanding the operating time, reducing the risk of expensive failures and allows several maintenance strategies. With different monitoring techniques, detailed informations about the transformer condition can be received and helps to minimize the probability of an unexpected outage. In this paper a methodology has been developed to use information derived from condition monitoring and diagnostics for rehabilitation purposes of transformers. The interpretation and understanding of the test data are obtained from the International Standards.
Inter-Coronary Communication: A Rare Anomaly in Unusual Site  [PDF]

Abstract: Intercoronary connection is a rare variant of coronary anomalies with a direct continuity between two main coronary arteries. It may function as an alternative pathway to blood flow in compromised coronary circulation. 64 years old male presented with 4 weeks history of retro-sternal chest pain and shortness of breath; his risk factors were diabetes mellitus, hypertension and smoking. Physical examination was normal and he had no audible murmur; he had elevated cardiac enzymes; in echocardiography there was inferior wall hypokinesia, cardiac catheterization revealed complete occlusion of med right coronary artery and complete occlusion of the left main (LM) trunk. Blood flow to the entire myocardium came from large abnormal coronary connection between the right coronary artery ostium and the proximal left anterior descending artery. A smaller arterial communication maintained blood flow to the distal right coronary artery. Cardiac computed tomographic angiography confirmed the presence of a patent left main ostium followed by complete occlusion. This unusual anatomical variation has saved the life of the patient and allowed time for surgical revascularization. The patient underwent successful triple coronary artery bypass grafts with no postoperative difficulties or complications. On one-year follow-up, the patient is asymptomatic, fully active with no new ECG or echocardiography changes.

Abstract: Background: Anomalies of the coronary arteries are uncommon, usually discovered incidentally during diagnostic cardiac catheterization or cardiac computed tomography (CCT). Coronary artery fistula (CAF) is one of the varieties of coronary termination anomalies. Some of CAF patients have symptoms of myocardial ischemia. Methods: In King Abdulaziz university hospital (KAUH) over six years period, five patients presented with acute coronary syndrome (ACS) and were diagnosed to have CAF by cardiac catheterization during the diagnostic workup of ischemic heart disease (IHD), four males and one female, mean age 57.4 ± 6.6 years; four patients had CAF with associated coronary atherosclerotic disease (CAD); one patient had large complex CAF with no significant CAD; all patients with CAD underwent closure/excision of the CAF with surgical revascularization; the patient with no CAD preferred surgical excision over endovascular intervention. Results: All patients had surgery with uneventful postoperative course and they were doing well with 3 to 7 years outpatient follow up. Conclusion: Surgical management is safe in patients with coronary artery anomalies presenting with ACS.
