Development of a patient-reported outcome scale for stable chronic obstructive pulmonary disease and its clinical applicability

Abstract: Objective: To develop a patient-reported outcome (PRO) scale for patients with stable chronic obstructive pulmonary disease (sCOPD) and to study its clinical applicability in traditional Chinese medicine (TCM).Methods: According to standardized procedures, the test version of PRO scale for sCOPD (sCOPD-PRO scale) was developed based on the theory of “unity of body and spirit” and was used to test 324 patients selected from six clinical centers. Five methods were used to analyze the items; the reliability, validity and responsiveness of the remaining items of the scale were evaluated. Results: (1) sCOPD-PRO consisted of 23 items after item analysis, and according to the content of each item, these items could be grouped into four domains, namely, lung and kidney deficiency symptom, spleen deficiency symptom, functional activities and emotional impact. (2) Cronbach’s alpha coefficient for sCOPD-PRO scale was more than 0.75 in four domains and total scores. (3) sCOPD-PRO scale and St. George’s Respiratory Questionnaire (SGRQ) were moderately correlated or highly correlated in corresponding domains: the correlation coefficient between the total score of sCOPD-PRO scale and the total score of SGRQ was 0.72 (P<0.01), the correlation coefficient between functional activities domain of sCOPD-PRO scale and activity domain of SGRQ was 0.77 (P<0.01), and the correlation coefficient between emotional impact domain of sCOPD-PRO scale and impact domain of SGRQ was 0.51 (P<0.01). Factor analysis revealed that four factors could explain 65.06% of the total variance. (4) The four domains and total scores of sCOPD-PRO scale were significantly different before and after intervention in TCM group and Western medicine group (P<0.01); moreover, the effect size and standardized response mean of two groups were more than 0.4. Conclusion: The development process and method of sCOPD-PRO scale, which is based on the theories of “unity of body and spirit”, are standard, and the reliability, validity and responsiveness of the scale are sound and can be relied upon for sCOPD clinical efficacy evaluation of TCM.

Abstract: Chemical investigation of the ethanol extract of the aerial parts of Scyphiphora hydrophyllacea Gaertn.collected in Hainan Province of China resulted in the isolation of a new iridoid, scyphiphin C (1) and a known iridoid glycoside, shanzhiside methyl ester (2). Their structures were elucidated by a study of their physical and spectral data.

Abstract: Chemical investigation of the EtOH extract of the twigs of Calophyllum inophyllum collected in Hainan Province of China resulted in the isolation of two new prenylated xanthones, caloxanthone O (1) and caloxanthone P (2). Their structures were elucidated by a study of their physical and spectral data. Compound 1 exhibited cytotoxicity against human gastric cancer cell line (SGC-7901),with an IC50 value of 22.4 μg mL-1.

Abstract: A new 2-(2-phenylethyl)chromone, 5,6,7,8-tetrahydroxy-2-(3-hydroxy-4-methoxyphenethyl)-5,6,7,8-tetrahydro-4H-chromen-4-one (1) was isolated from the Chinese eaglewood [Aquilaria sinensis (Lour.) Gilg]. Its structure was established by detailed MS and NMR spectroscopic analysis, as well as comparison with literature data.

Abstract: To study the antimicrobial components from the endophytic fungus A1 of mangrove plant Scyphiphora hydroph yllacea Gaertn. F ., a new fatty acid glucoside was isolated by column chromatography from the broth of A1, and its structure was identified as R-3-hydroxyundecanoic acid methylester-3- O-α-l-rhamnopyranoside ( 1) by spectroscopic methods including 1D and 2D NMR (HMQC, 1H- 1H COSY and HMBC) and chemical methods. Antimicrobial assay showed compound 1 possessed modest inhibitory effect on Saphylococcus aureus and methicillin-resistant S. aureus (MRSA) using the filter paper disc agar diffusion method.

Abstract: This article introduces the general outline of Dazhangtuo Underground Gas Reservoir (UGR) , summarizes and analyzes the mode of management and operation for this reservoir. From beginning of being putting into service at the end of 2000 till March of 2003, its sum total amount of gas injection is 0. 825 billion cubic meters, and of gas recovery is 0. 803 billion cubic meters. Its recent operation principle is as follows: I Absorption capacity of injection well is influenced by the effective permeability in the reservoir formation and average effective thickness; II On condition that injection pressure is able to resist the permeating from fluid, the fluid in low-pressure area in the gas reservoir (Banzhong) move slowly, thus UGR is expanded; III Its effective working gas amount is about 0. 5 billion cubic meters; IV As long as formation pressure is not below the minimum operation pressure of UGR, the production capacity of single well is up to 500-600 thousand cubic meters per day; V The interface between gas and water move outside along with pressure increase of UGR, and the gas reservoir have good flat connection; VI The effective working gas amount is little. At last, the article produces detailed operation methods how to make optimization of operation.
Stratified analysis of the relationship between traditional Chinese medicine constitutional types and health status in the general population based on data of 8 448 cases

Abstract: Objective: To examine the relationship between traditional Chinese medicine (TCM) constitutional types and health status among groups of different age or gender in the general population of China. Methods: Data of 8 448 cases were randomly sampled from a database of 21 948 cases of a cross-sectional survey on the TCM constitutional types and health status which was carried out in 9 provinces or municipalities of China (Jiangsu, Anhui, Gansu, Qinghai, Fujian, Beijing, Jilin, Jiangxi and Henan) according to gender and age structure of the Chinese population in 2005. Scores of health-related quality of life scale — the Medical Outcomes Study 36-Item Short-Form Health Survey (MOS SF-36) — were analyzed by Nemenyi test to compare the health status of individuals with different constitutional types. Results: Compared with the gentleness type, the MOS SF-36 scores of the 8 types of pathological constitution were significantly low (P<0.05) among groups of different age or gender. The MOS SF-36 score was the lowest in men of the qi-deficiency, qi-depression and blood-stasis types, while it was the lowest in women of the phlegm-dampness, qi-depression and qi-deficiency types. For the age group of 15 to 34, the special diathesis, qi-depression and blood-stasis types had the lowest MOS SF-36 scores; for the age group of 35 to 59, the qi-deficiency, qi-depression and blood-stasis types had the lowest MOS SF-36 scores; for the age group of over 60, the qi-deficiency, qi-depression and phlegm-dampness types had the lowest MOS SF-36 scores. Conclusion: In groups of different gender or age, the MOS SF-36 scores of the 8 types of pathological constitution were significantly lower than that of the gentleness type, indicating a deficient health status. The health status of different types of constitution showed different characteristics in groups of different gender or age.
Relationships between constitutional types of traditional Chinese medicine and hypertension

Abstract: Objective: To investigate the relationships between constitutional types of traditional Chinese medicine (TCM) and hypertension so as to provide epidemiological evidence for the theory of correlation between constitution and disease.Methods: A cross-sectional survey of TCM constitution data from a population of 7 782 from Beijng and 8 provinces of China (Jiangsu, Anhui, Gansu, Qinghai, Fujian, Jilin, Jiangxi, and Henan) was made in the study. The survey of TCM constitutions was performed by standardized TCM Constitution Questionnaire. Discriminatory analysis was used to judge the individual constitutional types including normal constitution, and qi deficiency, yang deficiency, yin deficiency, phlegm-dampness, damp-heat, blood stasis, qi stagnation and special constitutions. A multiple stepwise logistic regression analysis was applied to explore the significantly influential constitutional factors of hypertension.Results: After controlling several factors like gender, age, marital status, occupation, and educational background, three TCM constitutional factors according to different degrees of relative risks were entered into the multiple stepwise logistic regression model. The three factors were phlegm-dampness, yin deficiency and qi deficiency constitutions, and the odds ratio (OR) and 95% confidence interval (CI) were 2.00 [1.58, 2.55], 1.66 [1.33, 2.08] and 1.37 [1.13, 1.66] respectively. The main constitutional influential factors of hypertension in male patients were phlegm-dampness and yin deficiency constitutions, with OR and 95% CI of 1.61 [1.22, 2.14] and 1.60 [1.17, 2.19]. Phlegm-dampness, yin deficiency and qi deficiency constitutions were the main constitutional influential factors of hypertension in female patients. The OR and 95% CI were 2.80 [1.79, 4.39], 1.55 [1.13, 2.14] and 1.39 [1.05, 1.84] respectively. Phlegm-dampness constitution had more influence on hypertension in female patients than other constitution types.Conclusion: Phlegm-dampness, yin deficiency, and qi deficiency constitutions are the main influential factors of hypertension. Hypertensive patients with different gender have different constitutional influential factors.
Logistic regression analysis on relationships between traditional Chinese medicine constitutional types and overweight or obesity

Abstract: Objective: To explore the relationships between traditional Chinese medicine (TCM) constitutional types and overweight or obesity so as to provide evidence for adjusting constitutional bias and preventing and treating obesity.Methods: The data comes from a cross-sectional survey on TCM constitution of 18 805 samples aged above 18 in Beijing and 8 provinces (Jiangsu, Anhui, Gansu, Qinghai, Fujian, Jilin, Jiangxi and Henan) in China. The survey of TCM constitution was performed by standardized constitution in Chinese medicine questionnaire (CCMQ). Discriminatory analysis method was used to judge the individual’s constitutional type (gentleness type, qi-deficiency type, yang-deficiency type, yin-deficiency type, phlegm-dampness type, dampness-heat type, blood-stasis type, qi-depression type and special diathesis type). The relationships between TCM constitution types and overweight or obesity was investigated by logistic regression analysis. Results: Compared with gentleness type, the risk of overweight (OR, 2.05; 95% CI, 1.79-2.35) and obesity (OR, 4.34; 95% CI, 3.52-5.36) in phlegm-dampness type is significantly increased; the risk of obesity (OR, 1.60; 95% CI, 1.30-1.98) in qi-deficiency type is significantly higher; the risk of overweight and obesity in yang-deficiency type, blood-stasis type, and qi-depression type is significantly lower. Conclusion: Phlegm-dampness type and qi-deficiency type are the main constitutional risk factors of overweight or obesity.
Meroterpenes from Endophytic Fungus A1 of Mangrove Plant Scyphiphora hydrophyllacea

Abstract: Four new meroterpenes, guignardones F–I ( 1– 4), together with two known compounds guignardones A ( 5) and B ( 6) were isolated from the endophytic fungus A1 of the mangrove plant Scyphiphora hydroph yllacea. Their structures and relative configurations were elucidated by spectroscopic data and single-crystal X-ray crystallography. A possible biogenetic pathway of compounds 1– 6 was also proposed. All compounds were evaluated for inhibitory activity against methicillin-resistant Staphylococcus aureus (MRSA) and Staphylococcus aureus.
